1. The Subject Of The Order Includes The Provision Of Laundry Services For The Needs Of The Specialist Hospital. Edmund Biernacki In Mielec, Including: A) Washing, Disinfection, Drying, Ironing, Ironing Of Hospital Products, B) Using Washing Technology In Accordance With Sanitary And Epidemiological Supervision In Relation To Individual Hospital Departments, C) Receiving, Issuing, Returning And Recording Ordered Goods For Washing Hospital Items, D) Carrying Out Regular Repairs Of The Items (Including Darning And Patching, Sewing Broken Places, Replacing Buttons, Straps, Repairing Zippers, Etc.).2. The Quantity Of Hospital Products Ordered By The Ordering Party For Washing In One Month Is Up To 13,000 Kg, Including: A). Assortment: - General Hospital Up To 7,370 Kg - Surgical Up To 2,750 Kg - Neonatal And Pediatric Up To 500 Kg - Infectious Disease Wards Up To 1,400 Kg - Other (Towels, Blankets, Pajamas) Up To 480 Kgb) Mattresses, Work Clothes, I.E. Aprons, Suits And Other Clothes Up To 500 Kgc) Work Clothes (Green From The Operations Theater Will Be Counted And Confirmed By The Operations Theater Employees).3. The Ordering Partys Requirements Regarding The Subject Of The Order: A) The Hospital Assortment Intended For Washing Will Be Stored In The Hospitals Dirty Linen Storage Rooms, Packed In Gray Bags Provided By The Contractor. The Dimensions Of The Bags Will Be Agreed With The Ordering Party. The Bags With Infectious Products Will Be Marked With The Name Of The Department With The Words Contaminated Underwear In Red Marker. B) Dirty Items For Washing Will Be Collected By At Least Two Of The Contractors Employees, Between 7 A.M. And 9 A.M. The Contractor Will Weigh And Count Them. And Segregation Of The Assortment In The Presence Of The Ordering Partys Employee From The Hospitals Dirty Linen Warehouse. Based On The Above Data, The Hospital Employee Will Make An Entry In The Receipt (Laundry Order), Which Will Specify The Quantity And Weight Of The Items And Clothes To Be Washed, Specifying The Type Of Items To Be Washed, And Will Be Signed By Persons Authorized By The Ordering Party And The Contractor. The Receipt Will Be Prepared In Two Copies (One Copy Will Remain With The Ordering Party). The Assortment Will Be Weighed By Two Employees Of The Contractor (Unloading Dirty Laundry, Weighing And Unloading), In The Presence Of An Employee Of The Ordering Party. The Weight Of The Laundry Will Be Recorded On The Order Receipt. The Weight Determined In The Above Way Will Constitute The Basis For Settlement. Due To Space Limitations, The Assortment From The Dirty Warehouse Will Be Successively Taken From The Warehouse To The Contractors Car After Counting The Ward. Dirty Assortment From The Operating Theater, Delivery Tract, Infectious Disease Ward, Tuberculosis And Lung Diseases Ward And From Other Branches, If It Is Packed In A Bag Marked With The Red Inscription Contaminated Underwear (And In The Event Of A Pandemic, It Will Be Packed In Hydrobag-Type Dissolvable Bags), It Will Not Be Subject To Segregation - It Will Only Be Weighed And Handed Over To The Contractor Providing Laundry Services Without Opening. Contractors - After Collecting The Dirty Linen Found To Be With An Appropriate Receipt - There Is No Right To Make A Complaint.C) The Contractor Will Deliver Washed Hospital Products: • Packed In Disposable Bags, With Assortment Segregation, • Clearly Marked To Which Facility It Should Be Delivered, Including A Description Of The Packaging Contents.D) Clean Hospital Products, Sorted By Assortment (Pillowcases, Covers, Sheets, Etc.) Should Be Delivered To The Hospitals Clean Linen Warehouse No Later Than 9:00 A.M. (Or At The Hours Determined By The Mutual Schedule) Of The Day Following The Receipt Of The Assortment Intended For Washing. From 9:30 A.M., The Assortment Will Be Distributed By The Contractor, Divided Into Assortments Into Individual Hospital Facilities, In The Presence Of An Employee Of The Ordering Party, And The Quantity Will Be Written Off From The Receipt (Laundry Order) From Previous By A Hospital Employee. After Issuance, The Receipt Will Be Signed By A Representative Of The Contractor And The Ordering Party. The Ordering Party Requires That The Duration Of Laundry Services Does Not Exceed 24 Hours (In The Case Of Fashionable Clothes - 48 Hours, And In The Case Of Mattresses, Blankets And Pillows - 72 Hours). E) The Service Will Be Provided Every Day From Monday To Saturday Inclusive (With The Possibility Of Shortening It If There Is No Need To Perform The Service - Until Friday), And If There Is A Need To Provide The Service On Other Non-Working Days, The Contractor Will Be Informed One Week In Advance. The Ordering Party Requires The Contractor To Ensure The Continuity Of Laundry Services In Emergency Situations Occurring At The Contractors. F) The Contractor Will Ensure The Transport Of Hospital Supplies From The Dirty Linen Warehouse And To The Clean Linen Warehouse On Its Own. Trolleys Intended For Intra-Hospital Transport Of Goods Must Be Properly Maintained So As Not To Cause Environmental Nuisance (Noise, External Aesthetics, Etc.). The Goods Must Be Transported Taking Into Account The Division Into Clean And Dirty Transport. This Means That The Laundry Assortment Received From The Ordering Party Cannot Be Transported At The Same Time As The Clean Assortment, And The Trolleys After Transporting Dirty Linen Must Be Subjected To Documented Disinfection. G) The Contractor Is Fully Responsible For The Quality Of Chemicals, Ensures The Cleanliness Of The Washed Hospital Assortment, Care For Its Condition, Transport, Packaging And Delivery To The Place Indicated By The Ordering Party. If The Contractor Changes A Chemical Agent To One Other Than That Indicated In The Offer, He Will Be Obliged To Inform The Ordering Party About It And Submit A Document Authorizing The Use Of The Agent. All Chemical Agents Used By The Contractor Must Have Appropriate Approvals, Certificates And Certificates Authorizing Their Use In Service Facilities. Health. Disinfectants In The Case Of Contaminated Linen Must Have A Sporicidal Effect. H) Sterilization Of The Operational Assortment Will Be Provided By The Ordering Party, The Remaining Sterilization Of The Assortment Requiring Sterilization Is Provided By The Contractor. I) The Contractor Shall Bear The Costs Of Purchasing Washing Agents, Disinfectants And Bags, Containers And Other Necessary Items To Provide Laundry Services.J) The Ordering Party Will Have The Right To Make A Complaint In The Event Of Incorrect Performance Of The Service. The Ordering Party Will Return The Questioned Product, Which Will Be Marked Return. The Complaint Service Is Free Of Charge. The Contractor Is Obliged To Immediately Provide The Service Regarding The Reported Complaint - Within 24 Hours. K) The Ordering Party, In The Presence Of An Authorized Representative Of The Contractor, Will Carry Out Random Tests Of The Microbiological Purity Of Clean Bedding. If A Positive/Incorrect Result Is Obtained, The Costs Of The Test Will Be Covered By The Contractor Providing The Laundry Service.L) Once A Quarter, The Contractor Will Be Obliged To Provide The Ordering Party With Copies Of Microbiological Tests Performed In The Contractors Laundry, Confirmed By The Sanitary And Epidemiological Station, Documenting The Effectiveness Of Disinfection.M) Before Concluding The Contract The Ordering Party Reserves The Right To Verify The Data Provided In The Offer By Visiting The Contractors Laundry.N) The Contractor Will Appoint A Person Responsible For All Laundry Services Provided, Authorized To Make Arrangements And Contacts With The Ordering Partys Representatives.O) Laundry Services Will Be Provided In Accordance With Sanitary And Epidemiological Data For Disinfection And Washing Processes Depending On The Product Range, Contamination Of The Product Range And In Accordance With The Technology And Conditions Applicable In Health Care Facilities, Strictly Observing The Orders And Recommendations Of The Chief Sanitary Inspector. P) The Procedures Used Must Eliminate The Occurrence Of Links Or Sources Of Infection And Prevent Intra-Hospital Infections. The Contractor Will Be Responsible For The Performance Of The Laundry Service In Terms Of Quality And Compliance With Sanitary Requirements Towards The Control Authorities (State Sanitary Inspection). The Consequences Of Reservations And Decisions Issued By Sanitary And Epidemiological Stations Regarding Laundry And The Correctness Of Disinfection Will Be Borne By The Contractor.Q) The Ordering Party Will Have The Right To To Carry Out Inspections Regarding The Use Of Appropriate Cleaning, Washing And Disinfecting Agents By The Contractor.
