The Subject Of The Order Is The Provision Of Specialized Care Services For People With Mental Disorders, Clients Of The Municipal Social Welfare Center In Sicienko In 2024 Pursuant To Art. 50(7) Of The Act Of March 12, 2004 On Social Assistance (Journal Of Laws Of 2023, Item 901, As Amended) And The Regulation Of The Minister Of Social Policy Of September 22, 2005 (Journal Of Laws Of 2005 No. 189, Item 1598, As Amended) On Specialized Care Services.2. The Contractor Should Meet The Requirements Regarding The Security Of Information/Personal Data Processing, In Accordance With The Provisions Of The Processors Survey. 3. All Information Regarding The Protection Of Personal Data Is Included In The Following Annexes To The Swz, I.E.: - Annex No. 5 To The Swz - Questionnaire For The Processing Entity Before Concluding The Contract - Annex No. 6 To The Swz - Agreement Entrusting The Processing Of Personal Data4. The Contractor Undertakes To Provide Specialized Care Services For People With Mental Disorders For A Minimum Of 1,591 Hours, Which May Be Increased To A Maximum Of 3,025 Hours In The Scope Of: 1) Teaching And Developing Skills Necessary For Independent Living, In Particular: A) Developing The Skills To Meet Basic Life Needs And Social Functioning Skills, Motivating For Activity, Treatment And Rehabilitation, Conducting Self-Service And Social Skills Training And Providing Support, Also In The Form Of Assistance In Everyday Life Activities, B) Intervention And Assistance In Family Life, C) Assistance In Dealing With Matters Official, D) Support And Assistance In Obtaining Employment, E) Assistance In Managing Money. 2) Care - As Supporting The Treatment Process, Including: A) Assistance In Access To Health Services, B) Arranging And Keeping An Eye On The Dates Of Medical Visits And Diagnostic Tests, C) Assistance In Purchasing Or Ordering Medicines At A Pharmacy, D) Supervising The Intake Of Medicines And Observing Possible Side Effects Of Their Use, E) In Particularly Justified Cases, Changing Dressings, Assistance In The Use Of Medical Aids And Materials, Orthopedic Items, As Well As In Maintaining Hygiene, F) Assistance In Reaching Health Care Facilities, G) Assistance In Reaching Rehabilitation Facilities, 3) Physical Rehabilitation And Improvement Of Impaired Body Functions To The Extent Not Covered By The Provisions Of The Act Of August 27, 2004 On Health Care Services Financed From Public Funds (Journal Of Laws Of 2023, Item 605, As Amended): A) In Accordance With The Recommendations Of A Doctor Or A Specialist In The Field Of Physical Rehabilitation Or Physiotherapy, B) Cooperation With Specialists In The Field Of Psychological, Pedagogical And Educational - Therapeutic Support Aimed At Multilateral Activation Of The Person Using Specialist Services.4) Housing Assistance, Including: A) In Obtaining Apartment, Negotiating And Paying Fees, B) Organizing Minor Renovations, Adaptations, Repairs, Eliminating Architectural Barriers, C) Shaping Proper Relationships Of The Person Receiving Help With Neighbors And The Host Of The House. 5) Providing Children And Young People With Mental Disorders With Access To Rehabilitation Activities And Rehabilitation And Educational Activities, In Exceptional Cases, If It Is Not Possible To Obtain Access To The Classes Referred To In Art. 7 Of The Act Of August 19, 1994. On Mental Health Protection (Journal Of Laws Of 2023, Item 1972).5. Services Will Be Provided In The Place Indicated In The Administrative Decision Granting Specialist Care Services To The Person, From Monday To Saturday In Exceptional Situations On Non-Working Days, I.E. On Sundays And Holidays At Hours Agreed Jointly By The Therapist And The User Or His/Her Legal Guardian From 8.00 A.M. To 8.00 P.M. 6. A Therapeutic Hour Is 60 Minutes.
