1. The Subject Of The Order Is The Provision Of Specialized Care Services For People With Mental Disorders, Provided To The Charges Of The Municipal Social Welfare Center In Their Place Of Residence.2. Specialized Care Services For People With Mental Disorders Are Adapted To The Specific Needs Resulting From The Type Of Disease Or Disability.3. The Following Types Of Specialized Care Services For People With Mental Disorders Have Been Distinguished: 1) Teaching And Developing Skills Necessary For Independent Living, In Particular: A) Developing The Ability To Meet Basic Life Needs And Social Functioning Skills, Motivating For Activity, Treatment And Rehabilitation, Conducting Training Self-Service And Social Skills As Well As Support, Also In The Form Of Assistance In Everyday Life Activities, In Particular Such As: • Self-Service, Especially Performing Economic And Cleaning Activities, Including The Skills Of Maintaining And Running A Home, • Care For Hygiene And Appearance, • Maintaining Contacts With Family Members, Peers, At The Place Of Study And Work And With The Local Community, • Organizing And Spending Free Time Together, • Using The Services Of Various Institutions; B) Interventions And Help In Family Life, Including: • Help In Coping With Situations Crisis - Specialist Counseling, Crisis Interventions, Psychological Support, Therapeutic Conversations, • Facilitating Access To Education And Culture, • Counseling, Coordination Of Activities Of Other Services For The Family Of Which The Person Receiving Help In The Form Of Specialist Services Is A Member, • Shaping Positive Relationships Of The Supported Person With Close People, • Cooperation With The Family - Shaping Appropriate Attitudes Towards A Sick Or Disabled Person; C) Assistance In Dealing With Official Matters, Including: • Obtaining Social, Pension And Disability Benefits, • Filling Out Official Documents; D) Support And Assistance In Obtaining Employment, In Particular: • Searching For Information About Work, Assistance In Finding Employment Or Alternative Work, In Particular Participation In Occupational Therapy Workshops, Professional Activity Centers, Community Self-Help Centers, Social Integration Centers And Clubs, Job Clubs, • In Completing Documents Needed For Employment, • In Preparing For An Interview With The Employer, Supporting And Assisting In Contacts With The Employer, • In Solving Mental Problems Resulting From Work Or Lack Of It; E) Assistance In Money Management, Including: • Learning How To Plan A Budget, Assisting With Expenses, • Assistance In Obtaining Fee Reductions, • Increasing The Ability To Manage Ones Own Budget And Becoming Financially Independent.2) Care - As Support For The Treatment Process, Including: A) Assistance In Access To Health Services, B) Agreeing And Keeping Track Of Appointments Medical Visits, Diagnostic Tests, C) Assistance In Purchasing Or Ordering Medications At The Pharmacy, D) Monitoring The Intake Of Medications And Monitoring Possible Side Effects Of Their Use, E) In Particularly Justified Cases, Assistance In The Use Of Medical Aids And Materials, Orthopedic Items, And Also In Maintaining Hygiene, F) Assistance In Getting To Health Care Facilities, G) Assistance In Getting To Rehabilitation Facilities.4. Physical Rehabilitation And Improvement Of Impaired Body Functions To The Extent Not Covered By The Provisions Of The Act Of August 27, 2004 On Health Care Services Financed From Public Funds: 1) In Accordance With The Recommendations Of A Doctor Or A Specialist In The Field Of Physical Rehabilitation Or Physiotherapy, 2) Cooperation With Specialists In The Field Psychological-Pedagogical And Educational-Therapeutic Support Aimed At Multilateral Activation Of The Person Using Specialized Services.5. Housing Assistance, Including: 1) Obtaining An Apartment, Negotiating And Paying Fees, 2) Organizing Minor Renovations, Adaptations, Repairs, Eliminating Architectural Barriers, 3) Shaping Appropriate Relationships Of The Person Receiving Assistance With Neighbors And The Owner Of The House.6. Providing Children And Adolescents With Mental Disorders With Access To Rehabilitation, Revalidation And Educational Activities.7. Specialized Care Services May Only Be Provided By Persons Qualified To Practice The Profession Of: Social Worker, Psychologist, Educator, Speech Therapist, Occupational Therapist, Nurse, Assistant To A Disabled Person, Community Caregiver, Specialist In Medical Rehabilitation, Physiotherapist Or Other Profession Providing Knowledge And Skills. Enabling The Provision Of Specific Specialized Services.8. Persons Providing Specialist Services For People With Mental Disorders Must Have At Least Three Months Of Professional Experience In Working With People With Mental Disorders, Gained In One Of The Following Units: 1) A Psychiatric Hospital, 2) In An Organizational Unit Of Social Assistance For People With Mental Disorders, 3) A Therapy Facility Or An Educational Facility Attended By Children With Developmental Disorders Or Mental Disabilities, 4) A Therapeutic, Educational And Upbringing Center, 5) A Rehabilitation Facility, 6) In A Unit Other Than Those Mentioned In Points 1) - 5) Providing Specialized Care Services For People With Mental Disorders.9. In Justified Cases, Specialized Services May Be Provided By People Who Acquire Or Improve The Required Professional Qualifications Referred To In Point. 7, Have At Least One Year Of Work Experience In The Units Referred To In Point 8 And Are Provided With The Opportunity To Consult With Persons Providing Specialist Services With The Required Qualifications. 10. Persons Providing Specialist Services For People With Mental Disorders Must Have Documented Training And Experience In The Ability To Develop Motivation For Behavior Accepted By The Environment, Develop Habits Of Purposeful Activity, And Conduct Social Behavior Training.11. Specialized Care Services Will Be Provided Based On An Administrative Decision.12. The Entity Providing The Services Will Each Time Receive A Written Order From The Ordering Party, Which Will Specify: 1) Place Of Service, 2) Duration Of The Service (Date And Number Of Hours), 3) Detailed Scope Of Activities To Be Performed.13. Settlement For The Services Provided Will Be Made Monthly On The Basis Of The Submitted Work Cards Of The Persons Providing The Services, Confirmed By The Signature Of The Person Under Care Or Another Person Indicated By Him.14. Estimated Number Of Hours During The Contract Period: 3,176 Hours.15. One Hour Of Services Is Worth 60 Minutes.16. Due To The Specificity Of The Order, Which Is Characterized By Variability Of Needs Due To The Changing Health Condition And Life Situation Of People Requiring Help - Beneficiaries Of The Municipal Social Welfare Center In Siemiatycze, The Ordering Party Reserves The Right To Use No Less Than 50% Of The Estimated Number Of Hours Referred To In Point 14. For This Reason, The Contractor Will Not Be Entitled To A Claim For The Unused Number Of Hours.17. The Contractor Is Obliged To Guarantee An Appropriate Number Of People Who Will Participate In The Execution Of The Order - At Least 10 People: (2 Physiotherapists, 1 Psychologist, 4 Educators, 1 Speech Therapist, 1 Speech Therapist, 1 Neurological Therapist) To Carry Out The Following Therapies: Biofeedback, Hand Therapy, Integration Sensory, Psychological, Art Therapy, Pedagogical, Speech Therapy, Physiotherapy, Music Therapy, Social Skills Training And Hippotherapy.18. The Contractor Is Obliged To Have Premises In The City Of Siemiatycze Enabling Contact Between The Ordering Party And Clients With The Coordinator Of Specialized Care Services For People With Mental Disorders And With Persons Employed By The Contractor During The Execution Of The Order;19. The Contractor Is Obliged To Provide One Person As A Coordinator Of Specialized Care Services For People With Mental Disorders.20. The Ordering Party Does Not Allow The Possibility Of Submitting Partial Offers.21. The Ordering Party Does Not Allow The Possibility Of Submitting Variant Offers.22. The Ordering Party Does Not Envisage An Electronic Auction, Nor Does It Intend To Conclude A Framework Agreement.23. The Ordering Party Does Not Envisage Exercising The Option Right.24. The Ordering Party Does Not Impose Any Employment Requirements Based On An Employment Relationship In The Circumstances Referred To In Art. 95 Of The Pzp.25 Act. The Ordering Party Does Not Impose Any Requirements Regarding The Employment Of Persons Referred To In Art. 96 Section 2 Point 2 Of The Act.26. The Ordering Party Does Not Impose Any Requirements Regarding The Possibility Of Applying For The Contract Only By Contractors Referred To In Art. 94.27. The Ordering Party Does Not Provide For Reimbursement Of The Costs Of Participation In The Procedure.28. The Ordering Party Does Not Provide For The Possibility Of Submitting Offers In The Form Of Electronic Catalogs Or Attaching Electronic Catalogs To The Offer.29. The Ordering Party Does Not Reserve On The Basis Of Art. 60 And 121 Of The Act, Personal Performance Of The Order By The Contractor. A Detailed Description Is Provided In Chapter Iv Of The Swz
