The Subject Of The Order Includes Services Tailored To The Specific Life Needs Of People Resulting From The Type Of Their Disease Or Disability, Provided By People With Specialized Professional Training. Above Services Are Provided In Accordance With Art. 18 Section 1 Point 3, Art. 50, 50A Of The Act Of March 12, 2004 On Social Assistance And The Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialist Care Services.1. The Contractors Tasks In The Field Of Specialized Care Services For People With Mental Disorders Tailored To The Specific Needs Of People Requiring Help In The Form Of Specialized Services Resulting From The Type Of Their Disease Or Disability, Provided By People With Specialized Professional Training, In Particular Include: 1) Teaching And Developing Skills Necessary For Independent Living, In Particular: A) Developing The Ability To Meet Basic Life Needs And Social Functioning Skills, Motivating For Activity, Treatment And Rehabilitation, Conducting Self-Service And Social Skills Training And Supporting, Also In The Form Of Assisting In Everyday Life Activities, Such Such As: - Self-Service, Especially Performing Economic And Cleaning Activities, Including The Ability To Maintain And Run A Home, - Care For Hygiene And Appearance, - Maintaining Contacts With Household Members, Peers, At The Place Of Study And Work And With The Local Community, - Organizing And Spending Time Together Free Time, - Using The Services Of Various Institutions, B) Interventions And Assistance In Family Life, Including: - Assistance In Coping With Crisis Situations - Specialist Counseling, Crisis Interventions, Psychological Support, Therapeutic Conversations, - Facilitating Access To Education And Culture, - Counseling, Coordination Of Activities Of Other Services For The Benefit Of The Family Of Which The Person Receiving Help In The Form Of Specialist Services Is A Member, - Shaping Positive Relationships Of The Supported Person With Close People, - Cooperation With The Family - Shaping Appropriate Attitudes Towards The Sick Or Disabled Person, C) Assistance In Dealing With Official Matters, Including: - Obtaining Social And Pension Benefits, - Completing Official Documents, D) Support And Assistance In Obtaining Employment, In Particular: - Searching For Information About Work, Assistance In Finding Employment Or Alternative Occupation, In Particular Participation In Occupational Therapy Workshops, Vocational Activity Centers, Community Self-Help Centers, Social Integration Centers And Clubs, Work Clubs, - In Completing Documents Needed For Employment, - In Preparation For An Interview With The Employer, Supporting And Assisting In Contacts With The Employer, - In Solving Mental Problems Resulting From Work Or Lack Of It, E) Assistance In Managing Money, Including: - Learning How To Plan A Budget, Assisting In Incurring Expenses, - Assistance In Obtaining Tax Relief, - Increasing The Ability To Manage Ones Own Budget And Financial Independence.2) Care - As Support For The Treatment Process, Including: A) Assistance In Accessing Health Services, B) Arranging And Keeping An Eye On The Dates Of Medical Visits And Diagnostic Tests, C) Assistance In Purchasing Or Ordering Medicines At A Pharmacy, D) Keeping An Eye On Taking Medications And Observing Possible Side Effects Of Their Use, E) In Particularly Justified Cases, Changing Dressings, Assistance In The Use Of Medical Aids And Materials, Orthopedic Items, As Well As Maintaining Hygiene, F) Assistance In Reaching Health Care Facilities, G) Assistance In Reaching Rehabilitation Facilities.3) Physical Rehabilitation And Improvement Of Disturbed Body Functions To The Extent Not Covered By The Provisions Of The Act Of August 27, 2004 On Health Care Claims Financed From Public Funds A) In Accordance With The Recommendations Of A Doctor Or A Specialist In The Field Of Physical Rehabilitation Or Physiotherapy, B) Cooperation With Specialists In The Field Of Psychological, Pedagogical And Educational-Therapeutic Support Aimed At Multilateral Activation Of The Person Using Specialized Services, 4) Housing Assistance, Including: A) In Obtaining An Apartment, Negotiating And Paying Fees, B) In Organizing Minor Renovations , Adaptation, Repairs, Elimination Of Architectural Barriers, C) Shaping Appropriate Relations Of The Person Receiving Help With Neighbors And The Host Of The House. 5) Providing Children And Adolescents With Mental Disorders With Access To Rehabilitation, Revalidation And Educational Activities, In Exceptional Cases, If They Are Unable To Obtain Access For Classes Specified In The Act Of August 19, 1994 On Mental Health Protection.2. Specialized Care Services For People With Mental Disorders May Be Provided On Working Days, Holidays And Non-Working Days At The Place Of Residence Or Temporary Stay Of The Person Who Has Been Granted Assistance In The Form Of Specialized Care Services For People With Mental Disorders.3. To Provide Specialized Care Services For People With Mental Disorders, The Contractor Is Obliged To Employ Physically And Intellectually Fit People Who Care About Personal Cleanliness And Hygiene, Are Able To Perform Physical Work, And Have The Ability To Maintain Interpersonal Contacts. The Contractor Undertakes That The Services Will Be Provided By People With Qualifications Specified In The Regulation Of The Minister Of Social Policy On Specialized Care Services.4. An Order To Provide Specialized Care Services For People With Mental Disorders For A Specific Person Will Be Submitted To The Contractor In Writing, Electronically Or By Phone. The Contractor Will Receive From An Employee Of The Municipal Social Welfare Center In Dąbrowa Górnicza Information Containing The Clients Data, I.E. Name, Surname, Address Of Residence Or Stay, Telephone Number (If Any), Scope Of Services Provided, Days Of Service Provision And Hours Of Service, Period Of Service Provision, I.E. The Start Date And End Date Of The Provision Of Services, The Clients Payment Per Hour Of Services Provided - According To Which The Client Will Be Covered By The Service Within The Periods And Hours Specified In The Administrative Decision.5. If The Contractor Intends To Employ A New Person To Provide Specialist Care Services For People With Mental Disorders From Outside The List Of People Submitted On The Day Of Signing The Contract, The Contractor Is Obliged To Supplement The Above List By Informing The Ordering Party In Writing About This Fact, Indicating The Name And Surname Of The New Person And His Qualifications. Professional. The New Person Must Meet The Requirements For Persons Providing Specialist Care Services For People With Mental Disorders Specified In Point. 3 And Must Be Employed On The Basis Of An Employment Relationship. Lack Of Immediate Response By The Ordering Party Regarding The Indicated Person Is Deemed To Be Acceptance In This Respect. In The Event Of Failure To Fulfill The Above Obligation, The Ordering Party Has The Right To Apply A Contractual Penalty In Accordance With § 9 Point 7 Of The Draft Contract.6. In Situations Particularly Justified By The Situation Of The Client Or The Contractor, It Is Allowed To Change The Person Providing The Services During The Validity Period Of The Decision Granting The Benefit To The Client After Prior Arrangement Of This Circumstance With The Ordering Party. In The Event Of A Change In The Person Providing Services To A Given Client, The Contractor Is Obliged To Inform The Ordering Party Electronically No Later Than On The Day Of Receiving The Information. The Replacement Person Must Meet The Requirements For Persons Providing Services Specified In The Swz. Lack Of Immediate Response By The Ordering Party Regarding The Indicated Person Is Deemed To Be Acceptance In This Respect.7. The Date Specified In The Administrative Decision Is Considered The Start And End Date Of The Provision Of Specialist Care Services For People With Mental Disorders. 8. In The Event Of Suspension Of Services (E.G. Due To A Stay In Hospital, A Trip To Visit Family, Placement In A Nursing Facility, Death Or Other Reason), The Service Provider Is Obliged To Immediately Report This Fact To The Ordering Party By E-Mail, But No Later Than Within 2 Days. - Ch Business Days From The Moment Of Suspension Of Services.9. If It Is Necessary To Resume The Service, The Service Provider Is Also Obliged To Immediately Notify The Ordering Party Of This Fact Electronically, But No Later Than Within 2 Business Days From The Date Of Resumption Of The Service.10. Immediately After Commencing The Provision Of Services, The Contractor Is Obliged To Inform The Ordering Party Electronically About The Days And Hours Of Provision Of Services (Schedule) And Each Time About Any Change Of Dates (Schedule) And Provide Information About The Person Who Will Perform These Services (Name And Surname).11. In Particularly Justified Cases (Emergency Situations), E.G. The Clients Return From Hospital, Sudden Deterioration Of Health Or Other Conditions, It Is Possible To Start Providing Services Urgently Before Issuing An Administrative Decision At The Request Of The Ordering Party, I.E. Ordering The Provision Of Services Electronically Or By Phone In Accordance With Point 4 Above. In Such A Case, The Contractor Is Obliged To Provide The Customer With A Service In Accordance With The Response Time Declared On The Offer Form, But No Longer Than 10 Hours. In Special Situations, With The Consent Of The Ordering Party, It Is Possible To Waive The Response Time Of Providing The Customer With The Service In Emergency Situations On An Urgent Basis In The Environment Indicated By The Ordering Party.
