The Contractor Will Provide Services Consisting In Performing Medical Activities In The Scope Of Participation In Occupational Health And Safety Committee Meetings And Social Reviews Of Service And Work Positions In Military Units And Institutions Supplied By 21Wog.8. The Contractor Will Keep A Register Of Admitted Soldiers/Dzsw Soldiers Of Ron Employees, Containing: Name And Surname, Pesel Number, Address, Name Of The Doctor Providing The Service, Date Of Examination And Other Necessary Data To Identify The Health Service Provided.9. The Contractor Will Send An Invoice For The Occupational Medicine Services Provided, Issued Separately For Each Military Unit And Institution, Along With A Legible List Of The Examined Soldiers, Dzsw Soldiers And Ron Employees With Their Pesel Number, The Type And Number Of Tests Performed, The Total Value Of The Tests Performed And The Total Number Of Tests Performed.
