Applicants Expecting To Register For More Than One Item, Should Submit Separate Applications For Each Item And Should Credit Rs. 1000.00 Per Each Item To Account No. 7042222 Maintained At The Bank Of Ceylon, New Town, Polonnaruwa In Favour Of The District Secretary, Polonnaruwa And The Receipt Obtained From The Bank Or Else The Amount Should Be Paid In Cash To The Shroff Of The District Secretariat On Working Days Between 08.30 Hrs. And 15.00 Hrs And The Receipt Obtained Should Be Attached To The Application. Applications Submitted With Cheques And Money Orders Will Be Rejected.
