1. The Subject Of The Order Are Construction Works Consisting In The Reconstruction Of Municipal Road No. 810652P In Izbice Along With The Renovation Of The Rainwater Drainage System. The Task Should Be Performed In Accordance With The Assumptions And Guidelines Contained In The Project Documentation, As Part Of The Task Specified In The Budget Resolution Entitled: Reconstruction Of The Municipal Road No. 810652P In Izbice In Accordance With The Scope And Provisions Resulting From This Swz Together With The Annexes.2. The Description Of The Subject Of The Order Was Developed For The Following Industries: 1) Road Industry, 2) Sanitary Industry - Storm Sewage System, 3. The Scope Of This Order Includes The Performance Of Construction Works In The Following Scope: 1) Scope Of Works On The Municipal Road No. 810652P In Izbice: The Works Consist In Particular In The Reconstruction Of Approximately 780.00 M Of The Municipal Road No. 810652P In Izbice In Order To Improve Driving Smoothness And The Safety Of All Participants Traffic. As Part Of The Project, A New Construction Of A 6.00 M Wide Bituminous Road Should Be Constructed. The Scope Of The Study Also Includes The Construction Of A 2.50 M Wide Pedestrian And Bicycle Route (On The Right Side Of The Road), Exits And A Street Sewer Made Of Concrete Paving Stones. Drainage Will Take Place On The Surface Through Longitudinal And Transverse Slopes To The Existing Street Inlets, And Then To The Existing Storm Sewage System. All Devices Of The Water Supply, Sanitary, Gas And Telecommunications Networks Should Be Adjusted And Related In Height To The Designed Road. Additionally, The Project Includes Renovation Of Elements Of The Existing Storm Sewage System. The Scope Of The Renovation Includes The Replacement Of The Storm Sewage System With Wells, Sewers, Street Inlets And The Replacement Of The Outlet To The Ditch, As Well As Desilting And Leveling The Ditch.2) Scope Of Works To Be Performed (Road Industry): A) Demolition Of Road Elements, Bituminous Surfaces, Concrete Blocks , Concrete Slabs, Road Slabs, Concrete Curbs, Concrete Footings, Edges, Sub-Base, B) Profiling And Compaction Of The Subgrade, C) Construction Of The Sub-Base, D) Construction Of The Road Surface From Bituminous Mass, E) Construction Of The Exit Surface From Gray Concrete Paving Stonesf) Execution Surface Of The Pedestrian And Bicycle Route Made Of Red Concrete Paving Stones, G) Height Adjustment Of The Existing Surface Made Of Concrete Cubes, H) Sowing The Roadsides With Grass, I) Setting Curbs And Concrete Edges, J) Construction Of Drive-On Curbs On Exits, K) Construction Of Footings For Curbs , Edges, Sewage,L) Treatment Of Wells With Trapezoidal Stones,M) Regulation Of Water Supply, Sanitary, Gas And Telecommunications Network Devices (Refer In Height To The Design Road),N) Laying Street Sewage From Gray Concrete Paving Stones,O) Road Marking In Accordance With Permanent Traffic Organization, Note: The Description Of The Subject Of The Order Indicates The Applicable Color Of Concrete Paving Stones For The Implementation Of The Task Entitled: Reconstruction Of Municipal Road No. 810652P In Izbice. Note: Change Of The Structure Of The Pedestrian And Bicycle Route. Due To The Changes Introduced In The Structure Of The Pedestrian And Bicycle Route, The Works Should Be Carried Out In Accordance With The Following Layers: Structure Of The Pedestrian And Bicycle Route: - Wearing Course Made Of Concrete Blocks, Thickness: 8 Cm, - Cement-Sand Bedding, Thickness. 5 Cm, - Basic Foundation Made Of An Unbound Mixture 0/63 Mm C90/3 Gr. 15 Cm, 3) Scope Of Works To Be Performed (Sanitary Industry - Renovation Of The Storm Sewage System): A) Dismantling A Well, A Pipeline With A Diameter Of 400 Mm, 160 Mm, B) Channels Made Of Pvc Pipes With A Diameter Of 400 Mm, 160 Mm, C) Inspection Wells, D) Construction Of Street Sewage Wells With A Diameter Of 500 Mm With A Sedimentation Tank, E) Construction Of A Reinforced Outlet To The Ditch, F) Vertical Adjustment Of Manholes For Sewer Manholes And Street Sewage Grates, 4. In Addition To The Construction Works Specified Above, As Part Of The Order, The Contractor Will Also Be Obliged To: 1) Perform Geodetic Measurements Along With An As-Built Inventory And A Table Of As-Built Measurements, Which Should Include: A) Length Of: Road, Pedestrian And Bicycle Route, Sewage, Curbs, Edges, Stops ,B) Area Of: Road, Pedestrian And Bicycle Routes, Exits, C) Lengths Of Individual Rainwater Drainage Networks (Ø 160, Ø 400 - Separately), Number Of Wells, Inlets, 2) Tests Of The Degree Of Compaction Of The Road Subgrade, Which After Approval By The Inspector Supervision Should Be Delivered To The Municipal Office Of The Rawicz Commune,3) The Construction Site Should Be Made Available To The Water Supply And Sewerage Plant In Rawicz In Order To Rebuild The Water Supply Network And The Sanitary Sewage Network,4) Preparation Of Changes To The Design Documentation, If Necessary,5) Based On Current Maps From The Surveying Center, Especially When Renovating The Network And Earthworks, 6) Construction Of Drive-On Curbs On Exits, 7) All Formalities And Costs Related To Tree Felling Are The Responsibility Of The Contractor, The Contractor Is Obliged To Perform Compensatory Plantings, 8) Construction Of A Lowered Curb 0-1 Cm Within Pedestrian Crossings And The Use Of Integration Cubes With A Width Of Min. 50 Cm Along The Entire Length Of The Pedestrian Crossing,9) Finishing The Wells With Trapezoidal Stones In The Road And Pedestrian And Bicycle Paths,10) Constructing Protective Pipes On Networks Or Connections That Have Not Been Inventoried, If Necessary,11) Developing A Design For Temporary Traffic Organization (Including Obtaining The Necessary Opinions And Arrangements). The Complete Project Should Be Submitted To The Municipal Office Of The Rawicz Commune,12) Introducing Temporary Traffic Organization By Installing Appropriate Signs And Informing Residents About The Planned Works And Traffic Difficulties (At Least 7 Days Before Installing Vertical Signs Regarding The Protection Of The Works, Based On The Approved Design Of The Temporary Traffic Organization, The Traffic Management Authority, The Road Manager And The Relevant Police Commander Should Be Notified Of The Commencement Of The Works, Providing The Date Of Installation Of The Signs And The Date Of Restoring Or Introducing Permanent Traffic Organization On The Road - The Contractor Will Prepare A Report On The Acceptance Activities). 13) In The Event Of Introducing Individual Stages Of Temporary Traffic Organization, The Contractor Is Obliged To Notify The Traffic Management Body, The Road Manager, The Relevant Police Commander And Residents At Least 3 Days Before Installing Vertical Signs Regarding The Security Of The Works, Based On The Approved Temporary Traffic Organization Design. 14) Introducing Permanent Traffic Organization By Installing Appropriate Signs And Informing Residents About The Planned Works And Traffic Disruptions (At Least 7 Days Before The Introduction Of Permanent Traffic Organization, Based On The Approved Project, The Traffic Management Authority, The Road Manager And The Relevant Police Commander Should Be Notified On The Date Of Restoration Or Introduction Of Permanent Traffic Organization On The Road - The Contractor Will Submit To The Municipal Office Of The Rawicz Commune A Report On Acceptance Activities, Signed And Approved By Representatives Of The Traffic Management Body And The District Police),15) Other Works Accompanying The Execution Of This Task.5. The Detailed Scope Of The Order, Constituting A Description Of The Subject Of The Order, Was Determined By: 1) Design Documentation With Annexes, 2) Technical Specifications For The Execution And Acceptance Of Construction Works, 3) Permanent Traffic Organization Design, 4) Bill Of Quantities Together With A Zero Cost Estimate, Constituting An Integral Part Of This Specification. Terms And Conditions Of The Order. The Above-Mentioned Documents Are Posted On The Website Of The Proceedings.6. Note: Lump Sum Settlement Of The Investment. The Scope Of Work To Be Performed Is Determined By The Attached Bills Of Quantities Together With Zero Cost Estimates. Bills Of Quantities / Zero Cost Estimates Are Indicative - They Are Only An Auxiliary Document For Determining The Offer Price. The Contractor Will Assess The Final Scope Himself Based On The Designs And The Recommended Site Visit. 7. Pursuant To Art. 95 Section 1 And 2 And Art. 438 Of The Public Procurement Law, The Ordering Party Requires The Contractor Or Subcontractor To Employ, Under An Employment Relationship, Persons Performing The Following Types Of Activities Related To The Execution Of The Order Indicated By The Ordering Party, If The Performance Of These Activities Involves Performing Work In The Manner Specified In Art. 22 §1 Of The Act Of June 26, 1974 - Labor Code: - Works Involving The Performance Of Road Works - Works Involving The Performance Of Sanitary Works8. The Indication Of Trademarks, Patents Or Origin, Source Or Specific Process That Characterizes Products Or Services Provided By A Specific Contractor That May Appear In The Design Documentation Together With Attachments, Technical Specifications For The Execution And Acceptance Of Works Or Cost Estimate Documentation Has Been Used Solely To Indicate The Assumed Standard Of The Adopted Solutions. And Constitute An Equivalence Condition For Alternative Solutions. It Should Always Be Assumed That Such An Indication Is Accompanied By The Words Or Equivalent. The Ordering Party Informs That It Allows The Submission Of Equivalent Offers In Terms Of The Indicated Trademarks, Patents Or Origin, Source Or Specific Process That Characterizes The Products Or Services Provided By A Specific Contractor, However, The Norms, Standards And Technical Parameters Of The Materials Required By The Ordering Party Must Be Maintained. , Which Will Guarantee The Implementation Of Works In Accordance With The Design Documentation And Annexes. The Presented Parameters Of The Subject Of The Order Constitute The Technical And Qualitative Minimum Expected By The Ordering Party And Will Constitute The Basis For The Assessment Of Possible Equivalent Offers. Equivalent Materials Offered By Contractors Must Have Parameters No Worse Than Those Indicated In The Tender Materials.
