The Subject Of The Order Are Construction Works Consisting Of The Expansion, Reconstruction And Change Of Use Of The Former School Building In Stary Bostów Located On Plots No. 181/3, 182, 183/2 (Pawłów Commune, Starachowice Poviat, Świętokrzyskie Voivodeship) For Health Care Purposes. The Existing Building Of The Former School Was Built In 1994. In Traditional Brick Technology, With 3 Above-Ground Floors: Ground Floor, First Floor And High Basements. The Roof Over The Main Body Of The Building Is Gable, Covered With Trapezoidal Sheet Metal. The Building Is Not Currently In Use. As Part Of The Expansion, An Elevator Shaft Will Be Added With Space For A Lift Enabling Access To The Ground Floor And First Floor For Disabled People. The Reconstruction Will Cover All Floors Of The Building. New Partition Walls Will Be Built, New Door Openings Will Be Built In The Existing Internal Walls, Some Window Openings In The External Walls And Door Openings In The Internal Walls Will Be Bricked Up, The Load-Bearing Internal Walls Will Be Partially Demolished, The External Stairs Will Be Completely Dismantled And Rebuilt, And One Of The Ventilation Chimneys Will Be Dismantled, New Ventilation Chimneys Will Be Built. The Floors Will Be Replaced Along With Insulation. All Window And Door Frames Will Be Replaced. Additional Insulation Of The External Walls Of The Above-Ground Floors And Foundation Walls Will Be Performed Along With Moisture-Proof Insulation, And The Roof Covering Of The Entire Building Will Be Changed. In The Basement There Will Be Space For Technical Rooms, A Cleaning Room, A Server Room, A Medical Waste Room, A Gas Boiler Room And Social Rooms For Employees With A Cloakroom. With Sanitary Facilities. On The Ground Floor, It Is Planned To Create A Public Area For Patients With A Waiting Room, A Vaccination Point, A Doctors Office And Patient Registration. Additionally, It Is Planned To Create A Pharmacy Point With Facilities And Toilets. For Patients, Children, Staff And The Cleaning Room. On The First Floor There Are: A Gynecological Office With Registration And A Sanitary Facility, A Treatment Room, A Dentists Office, An Ultrasound And Ecg Room, A Laboratory Collection Point, Two Medical Offices, A Toilet For Patients And A Cleaning Room. A List Of Basic Dimensions Characterizing The Facility After The Planned Expansion, Reconstruction And Change Of Use: 1. Building Area Of The Entire Building: 334.15 M21.1. Building Area Of The Planned Extension: 25.40 M2. Usable Area Of The Entire Building: 727.03 M22.1. Usable Area Of The Extended Part: 51.16 M23. Total Area Of The Entire Building: 996.85 M23.1. Total Area Of The Extended Part: 70.60 M24. Gross Volume Of The Entire Building: 2,942.15.M34.1. Gross Volume Of The Extended Part: 234.68 M3. A Photovoltaic Installation With An Installed Capacity Of 9.9 Kwp Will Be Located On The Roof Of The Building. Scope Of Works For The Entire Construction Project And The Order Of Implementation: Scope Of Works: Demolition Works; Earthworks; Foundation Works; Construction Of Basement Walls; Construction Of Ground Floor Walls; Construction Of The Ceiling Above The Ground Floor; Construction Of First Floor Walls; Construction Of The Ceiling Above The First Floor; Carrying Out Reconstruction; Construction Of The Roof Structure And Covering; Construction Of The Facade; Execution Of Internal Installations; Finishing Works. Sequence Of Implementation: Demolition And Relocation Of The Fence; Demolition Of Existing Surfaces; Execution Of Expansion And Reconstruction; Construction Of Gas Installations In The Ground; Access And Access, Greenery Arrangement. The Building Was Designed To Provide The Necessary Conditions For Use By Disabled People. The Subject Of The Order And Quality Standards Are Detailed In The Design Documentation, Technical Specification For The Execution And Acceptance Of Works (Stwior) And Bill Of Quantities. They Constitute An Integral Part Of This Swz. The Works Covered By The Subject Of The Contract Should Be Performed In Accordance With The Design Documentation And The Conditions Contained In The Swz, The Contract Template And With Applicable Regulations, Standards And Principles Of Technical Knowledge And Construction Skills, Due Diligence In Their Execution, Safety, Good Quality And Appropriate Organization. The Ordering Party Requires A Site Visit Within The Subject Of The Order For Contractors Interested In Submitting An Offer. The Purpose Of The On-Site Visit Is To Familiarize The Contractor With The Specific Nature Of The Subject Of The Order, Including The Conditions For Its Implementation. Each Contractor Should Arrange The Date Of The On-Site Visit Individually With The Contact Persons In Sw. Bearing In Mind That Visits Can Only Be Carried Out In The Presence Of The Ordering Partys Employees And On The Agreed Dates. In Advance. The On-Site Visit Can Only Take Place Before The Scheduled Deadline For Submitting Offers. People Who Come To The On-Site Visit Are Obliged To Have An Identity Document With Them. A Protocol Signed By The Parties Will Be Prepared From The On-Site Visit, Which Will Constitute Confirmation Of The On-Site Visit (The Protocol Template Is An Annex To The Swz). The Cost Of The On-Site Visit (Related To Travel To The Site, The Use Of Own Employees, Etc.) And The Responsibility For The Interpretation Of The Content Of The Information Obtained And Other Consequences Of The On-Site Visit Are Borne By The Contractors Themselves. Failure To Participate In The Contractors On-Site Visit Results In Rejection Of His Offer Pursuant To Art. 226 Section 1 Point 18 Of The Public Procurement Law. All Materials Used Should Comply With Applicable Standards And Have The Required Approvals And Certificates And Must Not Pose A Threat To The Hygiene And Health Of Users In Accordance With The Requirements Of The Act Of July 7, 1994. - Construction Law. Depending On The Materials Used, The Technology And Manufacturers Requirements Must Be Strictly Observed. Before The Final Acceptance, A Set Of Pzh Certificates Must Be Presented And Attached To The Acceptance Documentation. The Contractor Will Also Include In The General Costs: 1) Comprehensive Surveying Service, 3 Copies Of The As-Built Surveying Inventory (Including 1 Copy On An Electronic Medium). Format And Content Of Files In Vector Form. The File Format Is Dwg Or Dxf.2) Preparation Of As-Built Documentation.3) Obtaining A Positive Decision On The Occupancy Permit. The Ordering Party Requires The Contractor To Provide A Full Guarantee And Warranty On The Subject Of The Contract For A Minimum Period Of 36 Months For Construction Works, Counting From The Date Of Final Acceptance Of The Entire Subject Of The Contract. .Extension Of Min. The Guarantee And Warranty Period Is The Criterion For Evaluating The Offers. The Investment Is Co-Financed By The Government Fund For Polish Order: Strategic Investment Program.
