The Subject Of The Contract Is The Implementation Of Tasks In The Field Of Government Administration, Resulting From The Program Of Comprehensive Support For Families For Life, Constituting An Attachment To Resolution No. 189 Of The Council Of Ministers Of December 27, 2021 Amending The Resolution On The Program Of Comprehensive Support For Families For ycie (Official Journal Of Laws Of 2022, Item 64), Hereinafter Referred To As The Program, In The Scope Of The Implementation Of The Tasks Of The Leading Coordination, Rehabilitation And Care Center In The Area Of The Poviat, Specified In Article 90V Paragraph 4 Of The Act Of 7 September 1991 On The Education System ( I.E. Journal Of Laws 2022, Item 2030) Resulting From Task 2.4 Of The Program: Multi-Aspect And Comprehensive Assistance To A Disabled Child In The Period From 0 To Starting School And His Family. In 2023, Classes Will Be Conducted From The Date Of Concluding The Contract Until December 16, 2023. Recruitment Of Children For The For Life Program Will Be Carried Out By The Public Psychological And Pedagogical Counseling Center In Lubliniec, Acting As A Leading Coordination, Rehabilitation And Care Center. Recruitment Of Children Will Be Carried Out Immediately After The Conclusion Of The Contract For The Provision Of Services That Are The Subject Of These Proceedings And Will End When The Childs Parents/Guardians Provide The Documents Necessary To Qualify Them For The Program. The Number Of Hours To Be Worked In Individual Parts Of The Order Will Depend On The Number Of Reported Children And The Type Of Disability Diagnosed, With The Proviso That The Above-Mentioned Numbers Of Hours For Given Specialists Are Maximum Numbers. The Program Will Be Carried Out In The Following Years Until 2026. A Separate Contractor Selection Procedure. Part 1 2 - Provision Of Services Related To Conducting Early Child Development Support Classes By A Neurospeech Therapist. The Tasks Of A Neurologist Will Include: A. After Appointing A Team Of Specialists On The Basis Of The Childs Confirmed Disability, Determining The Directions And Schedule Of Actions Taken In The Field Of Early Support And Family Support. Meetings Will Be Held At The Public Psychological And Pedagogical Counseling Center In Lubliniec At Least Twice A Year After Agreeing The Date With The Program Coordinator. The Team Will Be Appointed By The Program Coordinator, B. Development And Implementation Of An Individual Program Of Early Development Support With The Child And His Family In The Part Intended For Neurologopedy, C. Assessing The Childs Progress And Difficulties In Functioning, Including Identifying And Eliminating Barriers And Limitations In The Environment That Hinder Its Activity And Participation In Social Life, D. Analyzing The Effectiveness Of Assistance Provided Under The Program To The Child And His Family, E. Introducing Changes To The Program In Accordance With The Current Needs Of The Child And His Family, F. Planning Further Activities In The Field Of Early Support.G. Number Of Hours In Part 1: In 2023 - Max. 216 Hours (Two Hundred And Sixteen Hours), H. Number Of Hours In Part 2: In 2023 - Max. 198 Hours (One Hundred And Ninety-Eight Hours), Neurologopeda - Required Qualificationsa) Completed Uniform Masters Or Postgraduate Studies In The Field Of Neurologopedics,B) Has Pedagogical Preparation,C) Has At Least 2 Years Of Professional Experience In Working With Children With Speech Development Disorders And Working With Young Children With Disabilities (0-6 Years Old).
