1. The Subject Of The Contract Is Reconstruction Of The Section Of The Road To Agricultural Land In Orłówko. The Scope Of Works Includes The Reconstruction Of The Road Located On Plot No. 31/6 In Orłówko, On A Section Of Approx. 391 M. In The Area Covered By The Documentation, The Existing Road Has A Surface Made Of Prefabricated Concrete Elements, And Partly Of Stone Pavement With A Variable Width Of About 3.00 To 6.00 M (At The Passing Loop). There Is A Water Supply Network And A Sewage System In The Right-Of-Way. The Designed Section Of The Road Is Intended To Be Made As A Surface Made Of Bituminous Mass. The Width Of The Bitumen Road Will Be 3.0 M. The Scope Of The Contract Includes The Execution Of The Following Construction Works: • Preparatory And Demolition Works, • Construction Of The Foundation For Sliding Elements - Concrete Slabs, • Relocation Of The Existing Pavement Made Of Concrete Slabs, • Vertical Adjustment Of The Existing Road Surfaces, • Execution Of Trenching Under The Structural Layers Of Individual Pavements, • Execution Of Resistors And Edges, • Execution Of Structural Layers And A Surface Layer Of Bituminous Mass, • Execution Of Two-Sided Shoulders, • Execution Of Individual Exits, • Execution Of Cleaning Works, • Execution Of Passing Passages Of Bituminous Mass, • Other, Resulting From The Design Documentation. Detailed Scope Of Works To Be Performed Are Specified In The Design Documentation, Technical Specification For The Execution And Acceptance Of Construction Works And Bills Of Quantities As Auxiliary Documents, Constituting Attachments To The Swz. The Contractor Should Treat The Bill Of Quantities Prepared By The Employer As An Information Document That He May Use, But Is Not Obliged To Do So.2. The Scope Of The Order Also Includes: 1) Comprehensive Execution Of Construction Works Based On The Design Documentation Entitled: Reconstruction Of A Section Of The Municipal Road In Orłówek On Plot No. 31/6 As Well As The Technical Specification For The Execution And Acceptance Of Construction Works, 2) Execution Of Construction Works In In Accordance With The Provisions Of The Act Of July 7, 1994. Construction Law, Executive Regulations To The Above-Mentioned Act, Other Applicable Legal Acts Relating To The Subject Of The Contract, Current Polish Standards And Industry Standards And Relevant Health And Safety And Fire Protection Regulations, 3) Performance Construction Works From Own Materials And Equipment. All Materials And Equipment Used To Perform The Subject Of The Contract Should Have Appropriate Certificates, Technical Approvals And Approvals And Must Meet The Necessary Requirements In Accordance With Polish Standards. New Materials Approved For Use In The Construction Industry Must Be Used To Complete The Investment, Meeting The Conditions Set Out In The Act On Construction Products Of 16 April 2014 And The Implementing Regulations To This Act, 4) Providing Geodetic And Geotechnical Services, 5) Development Of The Construction Site And Securing It, 6) Execution Of Temporary Works That May Be Needed During The Performance Of Basic Works, 7) Designation Of The Construction Site Or Other Places Where Basic And Temporary Works Are To Be Carried Out, 8) Presentation Of Relevant Documents Related To The Transport Of Waste Generated During The Implementation Subject Of The Agreement And Their Storage At The Waste Disposal Site, 9) Organizing And Managing The Construction Site In Accordance With The Design Documentation And Applicable Health And Safety Regulations, As Well As Ensuring Fire Protection Conditions. Specified In The Detailed Provisions, 10) Informing The Employer About The Date Of The Works To Be Covered And About The Date Of Acceptance Of The Disappearing Works. If The Contractor Fails To Inform The Employer About These Dates, He Is Obliged To Uncover The Works Or Make Openings Necessary To Examine The Works, And Then Restore The Works To Their Previous Condition, At His Own Expense, 11) Making The Construction Site Available To Other Contractors Indicated By The Employer During The Performance Of The Subject Of The Contract, ) Allowing Access To The Construction Site For Employees Of Construction Supervision Authorities, Who Are Responsible For Performing Tasks Specified In The Construction Law Act, And Providing Them With Data And Information Required By This Act, And Other Employees Whom The Employer Will Indicate During The Period Of Performance Of The Subject Of The Contract,13) In The Event Of Destruction Or Damage To The Works , Their Parts, Underground Utilities Located At The Site Of Works Or The Employers Property - Repairing Them And Bringing Them Back To Their Previous Condition, At Their Own Expense,14) Guarding The Property Located On The Construction Site From The Date Of Taking Over The Construction Site To The Date Of Final Acceptance,15) Organizing The Social And Technical Facilities Of The Construction Site In The Sizes Necessary To Perform The Subject Of The Contract, 16) Keeping A Construction Log And Taking Measurements Of The Number Of Works Performed, 17) Marking The Works, 18) Immediately Informing The Employer, But Not Later Than Within 3 Days From The Date Of Receipt Of The Information, About Any Irregularities And Threats Of Irregularities That May Have Any Impact On The Situation Of The Employer In Matters Covered By This Agreement,19) Marking Dangerous Zones, Securing The Construction Site And Preventing Access By Third Parties,20) Developing A Temporary Traffic Organization Plan For The Duration Of The Works, Which After After Obtaining An Opinion From The Road Manager, It Should Be Submitted Together With An Appropriate Application For Approval By The Road Traffic Management Authority,21) Notifying Network Administrators Of The Planned Start And Completion Date Of The Works And Carrying Out The Works, Obtaining Partial And Final Acceptances To The Extent Resulting From The Industry Agreements Made, If Necessary, 22) Execution Of Works In Accordance With The Schedule, 23) Securing Detours Enabling The Current Uninterrupted Communication In Accordance With Applicable Regulations, In Agreement With The Employer, 24) Participation In The Acceptance Of Works Subject To Covering And The Final Acceptance Of Works, 25) The Contractor Undertakes To Provide Construction, Within The Limits Provided By The Employer, Proper Order, Compliance With Health And Safety Regulations, Protection Of Facilities And Networks As Well As Utilities Located On The Site And Keep Them In Proper Technical Condition, And Tidy The Area After Completion Of Construction,26) Organizing Coordination Meetings With Representatives Of The Employer And Other Invited Persons. The Purpose Of The Coordination Meetings Will Be To Discuss Current Issues Regarding The Execution And Progress Of The Works. The Dates Of The Coordination Meetings Will Be Determined By The Contractor. The Frequency Of Coordination Meetings Will Be Determined By The Parties To The Agreement. The Meetings Will Be Conducted And Recorded By The Employer, And Copies Of The Minutes Will Be Delivered To All Persons Invited To The Meeting; Implementation Of These Instructions, 28) Preparation Of As-Built Design Documentation At Own Expense, 29) Preparation Of The Final Settlement Of Works, 30) Completion And Presentation To The Employer Of Documents Allowing For The Assessment Of The Correct Performance Of The Subject Of Acceptance Of Works, Including The Geodetic And As-Built Inventory, 31) Employment Of A Sufficient Number Of Qualified Personnel Guaranteeing The Proper Quality Of The Work Performed,32) Execution And Assembly Of A Standing Information Board About Financing The Task From The Budget Of The Pomeranian Voivodeship With Dimensions Of 180X120 Cm, After Agreeing With The Ordering Party The Location Of The Board. The Board Design Template Is Not Subject To Modification And Will Be Provided To The Contractor After Signing The Contract.
