Hostel Service For Patients Of The Radiotherapy Center 1. Availability Of Hostel Services 365 Days A Year For 49 Patients Each Time.2. Rooms: 1, 2, 3 Persons Intended Exclusively For Patients Of The Radiotherapy Center Of The Specialist Hospital. A. Soko owski In Wa brzych, Hereinafter Referred To As The Guest. 3. The Distance Of The Facility From The Hospital Should Be A Maximum Of 9 Km, Counted In A Straight Line (As A Circle Radius) Of Soko owskiego Street, Given In Accordance With The Measure Of The Online Electronic Map (E.G. Maps.Google.Pl; Www.Zumi.Pl; Docelu.Pl; Mapa .Targeo.Pl Etc.), Road Distance Up To 10 Km Calculated From Soko owskiego.4 Street. All Accommodation Places Do Not Have To Be Located In One Facility At One Address.5. The Price Includes: A. Providing Their Own Set Of Keys For Each Guest, Ensuring Full Independence (Including A Key From The Common Room Available 24/7; It Allows To Offer Hotel Services Without Providing A Common Room But With Access To Tv In Each Apartment; E. Providing Accommodation Rooms With A Minimum Temperature Of 23 c In The Months From October To April; F. Ensuring The Possibility Of Opening Windows For Ventilation (Windows And Doors Are Efficient) Ensuring That The Entrances To The Building Where The Guest Accommodation Will Be Located Will Be Secured With A Lock Opened With A Key Or An Intercom; H. The Ordering Party Reserves The Right (During The Term Of The Contract) To Visit The Facility In Order To Check The Services Provided And Comply With The Required Conditions.11. The Ordering Party Requires The Contractors Statement On Meeting The Sanitary And Epidemiological Safety, Fire Safety And Health And Safety Conditions When Performing The Subject Of The Contract.12. Throughout The Duration Of The Contract, The Contractor Shall Have A Civil Liability Insurance Covering His Business Activity13. The Contractor Shall Keep Records Of Accommodation In A Manner Consistent With The Accommodation Allocated By The Radiotherapy Center Secretariat14. The Contractor Will Provide Hotel Services In Accordance With The Act Of August 29, 1997 On Tourist Services (Journal Of Laws Of 2014, Item 196 And 822, As Amended) In The Field Of Short-Term, Generally Available Rental Of Houses, Flats And Rooms.
