The Subject Of The Contract Includes In Particular: 1) Disassembly And Removal With Disposal Of System Walls. 2) Installation Of System Walls In Segment C (New Hpl Walls In The Possession Of The Employer). 3) Delivery And Assembly Of Hpl System Walls In Segment B. 4) Demolition Of Floors Of Tiles, Screeds - Removal, Disposal And Disposal. 5) Demolition Of Wall Linings And Plasters - Removal, Disposal And Disposal. 9) Building Partition Walls. 10) Disassembly, Delivery And Assembly Of Door Woodwork. 11) Implementation Of Horizontal Insulation. Replacement Of Radiators With New Ones.20) Delivery And Installation Of Dispensers, Dryers, Toilet Paper Containers (Not For Sheets), Waste Bins, Urinal Partitions, Hpl Walls, Electric Heaters (In Brushed Stainless Steel Version: Dryers With Adjustable Speed) .21) Execution Of Electrical Works: New Installations For Lighting Fittings (Separate Circuit), For Plug Sockets With Switches, Fittings And Motion Sensors (Separate Circuit) (With Switchgear For Each Complex Of Rooms). 22) Execution Of New Water Installations. Can. From The Risers One Storey Lower And Higher (Including New Risers On The Storey).23) Delivery And Assembly Of Fittings And White Assembly.3. All Works Related To The Renovation Of Bathrooms Must Be Carried Out Under The Direction And In The Presence Of An Authorized Construction Works Manager, Authorized At Least To A Limited Extent To Manage General Construction Works In The Construction And Construction Specialty. The Contractor Shall Be Obliged To Inform The Ordering Party About The Designated Works Manager Before Signing The Contract.4. Specification And Description Of Temporary Works And Accompanying Works: - Securing The Work Area Against Access By Third Parties And Against Building Users, - Ensuring The Safety Of Work Users And Bystanders In The Area Of Work Performed In Accordance With Health And Safety Regulations, - Presenting A Safety Certificate At Each Request Of The Employer, Compliance Or Certificate Of Compliance With The Polish Standard Or Technical Approval In Relation To The Indicated Materials Or Devices.5. The Contractor Is Obliged To Perform The Order On The Terms And Conditions Described In The Swz As Well As In The Draft Provisions Of The Contract Constituting Annex No. 3 To The Swz, Taking Into Account The Following Conditions For The Implementation Of The Task: B) The Works Will Be Carried Out In An Active Facility (Building A Will Not Be Out Of Use For The Duration Of The Works). C) The Ordering Party Specifies Working Days, Ie From Monday To Friday. The Detailed Scope Of The Contractors Works Is Specified In The Following Documentation: A) Design Documentation Constituting Annex No. 2 To The Swz, Including: 1) Drawings, 2) Technical Specifications For The Execution And Acceptance Of Construction Works: - Works In The Field Of Internal Electrical Installations, - Sewage Systems Made Of Pipes Made Of Plastics, - Plastic Water Supply Systems, - General Requirements, - Ceramic Cladding And Waterproofing In Wet Rooms, - Painting Works, - Masonry Works, - Covering Floors And Walls.3) Scope Of The Order - Description.B) Bill Of Quantities - Appendix No. 11 To Swz.6. The Employer Requires That The Subject Of The Contract Be Covered By A Minimum 12-Month Warranty From The Date Of Preparation Of The Final Acceptance Report For Construction Works Without Reservations. The Warranty Period Is A Criterion For Evaluating Offers. 7. The Contracting Authority Informs That Whenever The Subject Of The Contract Is Described In The Swz And Its Annexes: 1) By Reference To The Standards, Technical Assessments, Technical Specifications And Technical Reference Systems Referred To In Art. 101 Sec. 1 Point 2 And Par. 3 Of The Public Procurement Law, It Is Assumed That Solutions Equivalent To Those Described Are Allowed, And Such A Reference Is Accompanied By The Words Or Equivalent. Pursuant To Art. 101 Sec. 5 Of The Public Procurement Law, The Contractor Who Refers To Equivalent Solutions (In A Situation Where The Description Of The Subject Of The Contract Refers To The Standards, Technical Assessments, Technical Specifications And Technical Reference Systems Referred To In Article 101(1)(2) And (3) Of The Public Procurement Law) , Is Obliged To Prove In The Offer That The Solutions It Offers Meet The Requirements Set Out In The Swz And Annexes To The Swz To An Equivalent Extent. .3) The Obligation To Prove Equivalence Lies With The Contractor At The Time Of Submitting The Offer. The Contractor Shall Mark The Appropriate Option In Point 5 Of The Tender Form. If The Contractor Proposes Equivalent Solutions, The Contractor Submits A List Of Equivalent Solutions With The Offer And Attaches Evidence Of Equivalence. If The Contractor Does Not Mark Any Option In Point 5 Of The Tender Form, The Ordering Party Will Assume That At The Stage Of Order Execution The Contractor Will Apply The Solutions Described In The Swz And Annexes To The Swz.8. The Ordering Party Specifies The Following Requirements Regarding The Employment By The Contractor Or Subcontractor Of Persons Performing Activities Indicated By The Ordering Party In The Scope Of Order Performance Under An Employment Contract: The Ordering Party Requires Employment By The Contractor Or Subcontractor Under An Employment Contract, In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974 - Labor Code (Journal Of Laws Of 2020, Item 1320, As Amended), Persons Performing Activities In The Field Of Implementation By All Manual Workers And Operators Of Machines And Devices: - Construction Works Related To From Construction Works Related To Demolition Works (Dismantling), Masonry Works, Insulation, Plastering, Laying Tiles On Floors, Facing Tiles On Walls, Making Paint Coatings, Making Suspended Ceilings, Mounting Countertops And Mirrors, Mounting Hpl Walls, Mounting Fittings And Accessories Bathrooms, Electrical Installations, Water And Sewage Installations, Replacement Of Doors. The Above-Mentioned Requirement Also Applies To Subcontractors Performing The Above-Mentioned Works. The Requirement Of Employment Under An Employment Contract Does Not Apply To Subcontractors Conducting Business Activity Based On An Entry In Ceidg Or Other Equivalent Registers, Who Perform Personally And Independently Activities Entrusted To Them In The Implementation Of The Subject Of The Contract. 1 Of The Labor Code, Referred To Above, The Contractor Is Obliged, Prior To The Commencement Of Work By These Persons, To Provide The Employer With Documents Confirming Their Employment Under An Employment Contract, I.E.: 1) A Statement Of An Employed Employee On Employment By The Contractor Or Subcontractor Under An Employment Contract; 2) Statements Of The Contractor Or Subcontractor On The Employment Of An Employee Under An Employment Contract; 3) A Certified True Copy Of The Employment Contract Of An Employed Employee; 4) A Statement Signed By The Contractor Or Subcontractor Containing Information, Including Personal Data, Necessary To Verify Employment On Under The Employment Contract, In Particular The Name And Surname Of The Employed Employee, The Date Of Conclusion Of The Employment Contract, The Type Of Employment Contract And The Scope Of The Employees Duties. At The Same Time, The Ordering Party Informs That In The Draft Provisions Of The Contract In § 3 It Provided For The Ordering Partys Rights To Control The Contractors Fulfillment Of The Requirements Referred To In Art. 95 Sec. 2 Item 3 And Sanctions For Non-Compliance With These Requirements. Site Inspection: 1. Due To The Specificity Of The Subject Of The Contract, The Contracting Authority Recommends Submitting An Offer After The Site Inspection.
