The Subject Of The Contract Is The Implementation Of Tasks In The Field Of Government Administration, Resulting From The For Life Program Of Comprehensive Support For Families, Constituting An Attachment To Resolution No. 189 Of The Council Of Ministers Of December 27, 2021 On The For Life Program Of Comprehensive Support For Families (M.P. 2022 Item 64) Hereinafter Referred To As The Program In The Scope Of The Implementation Of The Tasks Of The Leading Coordination, Rehabilitation And Care Center In The Area Of The Lublin Poviat, Specified In Art. 90V Sec. 4 Of The Act Of September 7, 1991 On The Education System (Journal Of Laws Of 2023, Item 900, I.E.) Resulting From Task 2.4, Priority Ii Of The Program: Multi-Faceted And Comprehensive Assistance To A Disabled Child In The Period From The Age Of To Start Learning At School And His Family For The Psychological And Pedagogical Clinic In Be yce, Ul. Fabryczna 2B, 24 200 Be yce, By Specialists In The Field Of Psychology. The Program Is Addressed To Children From The Age Of 0 Until They Start School, Who Have Been Diagnosed With A Disability Or A Threat Of Disability. Services May Be Provided At The Psychological And Pedagogical Counseling Center In Be yce, Ul. Fabryczna 2B. Up To 5 Hours A Week For A Given Child, Depending On Their Needs. Classes Will Be Held In Accordance With The Number Of Hours Allocated To The Child, Determined By The Diagnostic Team Selected By The Director Of The Psychological And Pedagogical Clinic In Be yce, Ul. Fabryczna 2B, From Among Specialists Qualified For The Implementation Of The Program, According To The Agreed Schedule Of Classes. The Deadline For The Service Is December 9, 2023. Classes Are Conducted Individually With The Child And His Family. As Part Of Psychological Classes, In Order To Develop Social And Communication Competences Necessary For A Child To Function In Social Life, Classes Can Be Conducted In A Group, With The Participation Of Families Of Children Or Other Children Covered By Support. The Number Of Children In The Group Cannot Exceed 3 (This Applies To Tus Social Skills Training Classes). Classes In The Group Are Organized Depending On The Developmental Needs Of The Child In Accordance With The Indications Of The Consulting Team. The Contractor May Entrust The Performance Of Part Of The Contract To A Subcontractor. The Contracting Authority Understands 60 Minutes As An Hour. The Ordering Party Points Out That The Number Of Hours Of Providing Services Should Be Understood Only As The Actual Time Of Providing Services To Or For The Child. The Costs Resulting From The Scope Of The Subject Of The Order Are Covered By The Contractor As Part Of The Remuneration Specified In The Offer, Constituting The Maximum Obligation Of The Ordering Party. The Working Time Of Individual Specialists Will Be Determined In Accordance With The Psychophysical And Developmental Needs Of Children And Their Families. The Employer Allows For The Possibility Of Reducing The Number Of Hours Allocated To Contractors By Not Less Than 50% Of The Basic Number Of Hours Specified In The Contract. Failure By The Employer To Allocate To A Given Contractor The Full Number Of Hours Specified In The Contract Will Not Result In The Contractors Claim For Performance And Payment Of Remuneration For The Remaining Number Of Uncompleted Hours. The Contract Covers The Implementation Of The Subject Of The Contract In The Amount Of 203 Hours. Responsibilities Of The Psychologist: Conducting An Initial Diagnosis, Participation In The Meeting Of The Consulting Team Referred To In The Regulation Of The Ministry Of National Education Of August 24, 2017. On Organizing Early Support For Childrens Development (Journal Of Laws Of 2017, Item 1635), Developing And Implementing An Individual Development Support Program With The Child And His Family, In The Part Intended For The Psychologist, Assessing The Childs Progress And Difficulties Once A Year Within The Scope Of The Activities They Conduct, Analyzing The Effectiveness Of Assistance Provided Under The Program To The Child And His Family, Introducing Changes To The Program In Accordance With The Current Needs Of The Child, Planning Further Activities In The Field Of Supporting Development, Organizing Meetings And Consultations For The Childs Parents, Legal Guardians Of The Child, The Childs Actual Guardians (Their Number May Not Exceed 15% Of The Total Number Of Hours Spent With A Given Child), Keeping The Required Documentation (Recommendations Of The Consulting Team, The Log Of Specialist Activities, The Childs Individual File), Developing And Submitting A Monthly And Annual Report In Due Time In Addition: Determining The Level Of The Childs Functioning On The Basis Of The Diagnosis Contained In The Opinion On The Need For Early Support For The Childs Development, And In The Absence Of Such An Opinion, On The Basis Of An Individual, Initial Diagnosis In Accordance With The Qualifications Held, Based On Information On The Detection Of A Threat Of Disability Or Disability, The Direction, Scope And Schedule Of Proposed Activities In The Field Of Early Support And Support For The Childs Family, Or Other Specialist Activities Taking Into Account The Development Of The Childs Activity And Participation In Social Life And The Elimination Of Barriers And Limitations In The Environment That Hinder Its Functioning; Cooperation With The Childs Family, In Particular By: Providing Assistance In Shaping The Attitudes And Behaviors Desired In Contacts With The Child: Strengthening The Emotional Bond Between Parents And The Child, Recognizing The Childs Behavior And Consolidating Appropriate Reactions To These Behaviors, Providing Instructions And Advice, And Conducting Consultations On Working With The Child, In The Absence Of An Opinion On The Need For Early Support For The Childs Development, Cooperation With The Appropriate Psychological And Pedagogical Counseling Center And Providing Assistance To Parents In Order To Conduct Tests And Establish A Specialist Diagnosis Of The Childs Functioning Level; Development And Implementation Of A Weekly Schedule With The Child And His Family; Implementation, Within The Scope Of His Qualifications, Of Other Tasks Of The Coordination-Rehabilitation-Care Center Resulting From The Needs Of The Child And His Family, Including In Particular: Forms Of Specialist Assistance, Establishing Cooperation With A Kindergarten, Another Form Of Pre-School Education Attended By The Child Or Other Entities Where The Child Is Subject To Therapeutic Interactions In Order To Ensure The Consistency Of All Interactions Supporting The Childs Development; Establishing Cooperation With A Medical Entity In Order To Diagnose The Childs Needs Resulting From His Disability, Provide Him With Medical And Rehabilitation Support And Recommended Medical Devices, As Well As Advice And Consultations On Supporting The Childs Development; Establishing Cooperation With Organizational Units Of The Family Support And Foster Care System; Analyzing The Effectiveness Of Assistance Provided To The Child And His Family, Making Changes To The Program According To The Needs Of The Child And His Family, And Planning Further Actions. Keeping The Following Documentation: A) An Individual Program That Specifies In Particular: - The Method Of Achieving Developmental Goals Aimed At Improving The Childs Functioning, Strengthening Its Participation In Social Life And Preparing It For School Learning, Eliminating Barriers And Limitations In The Environment That Hinder The Childs Functioning, Including Including His Activity And Participation In Social Life; - Support For The Childs Family In The Implementation Of The Programme; - How To Assess The Childs Progress; B) The Childs Individual File, Which Includes: - Parents Application For The For Life Program; - Interview Questionnaire With Parents; - Indications Of The Consulting Team; - Medical Documentation; - Certificates On Special Education, Opinion On The Need For Early Development Support (If The Child Has Such); - Psychological, Pedagogical And Speech Therapy Documentation; - Other Relevant Documents; C) A Register Of Individual Classes With The Parents Signature Confirming Their Implementation; D) Monthly And Annual, Or In The Case Of Early Completion Of Classes, The Final Report On The Conducted Classes By December 15 For The Year, Including In Particular: - Description Of The Task Performance, Detailing The Completed Activities; - Tangible, Numerical And Qualitative Effects Of The Implemented Activities.
