1. The Subject Of The Contract Is The 24-Hour Collection And Transport Service Of Corpses, I.E. Bodies Of Deceased People And Bodies Of Stillborn Children (Hereinafter Also Referred To As Fetuses) From All Spsk-2 Units In Szczecin At Al. Powsta c w Wielkopolskich 72 To The Dissecting Room And The 24-Hour Collection And Transport Of Human Remains, Which Are Parts Of The Human Body Disconnected From The Whole (Hereinafter Also Referred To As Limbs) From The Ordering Partys Integrated Operating Block To The Cold Store. Both The Prosectorium And The Cold Store Are Located On The Premises Of The Ordering Party.2. The Estimated Number Of Transports Of Corpses, Fetuses And Limbs Has Been Included In The Unit Price Form (Annex 2 To Swz, Hereinafter Also Referred To As Fcj).3. The Ordering Party Reserves The Right To Order A Smaller Number Of Transports Than The Estimated One - But Not Less Than 60%. 4. A Detailed Description Of The Requirements Regarding The Subject Of The Contract And The Conditions For Its Implementation Is Contained In Chapter Ii Swz.5. The Order Consists Of 1 Task. 6. The Ordering Party Does Not Allow Variant Offers.7. Determining The Requirements Regarding The Employment By The Contractor Or Subcontractor Of Persons On The Basis Of An Employment Relationship: A) Pursuant To The Requirement Contained In Art. 95 Of The Public Procurement Law, The Contracting Authority Requires That Persons Performing Activities Resulting From The Performance Of The Contract During The Performance Of The Contract, Included In The Detailed Description Of The Subject Of The Contract (Chapter Ii Of The Swz) Are Employed By The Contractor Or Subcontractor On The Basis Of An Employment Contract;8. The Ordering Party Requires The Contractor To Have A Civil Liability Insurance In The Field Of Business Activity, Which Is The Subject Of This Contract, Covering Contractual And Tort Liability For Property Damage And Personal Damage And Their Consequences For The Guarantee Sum Not Lower Than Pln 100,000. The Ordering Party Requires The Performance Of The Subject Of The Contract For A Period Of 24 Months, Counted From The Date Of Commencement Of The Provision Of Services. The Approximate Date Of Commencement Of The Service Is October 1, 2023.
