The Subject Of The Contract Is Construction Works Consisting In The Extension, Reconstruction And Change Of Use Of The Former School Building In Stary Bostow Located On Plots No. The Existing Building Of The Former School Was Constructed In 1994. In Traditional Brick Technology, With 3 Overground Storeys: Ground Floor, First Floor And High Basements. The Roof Over The Main Body Of The Building - Gable, Covered With Trapezoidal Sheet. The Building Is Currently Not In Use. As Part Of The Extension, An Elevator Shaft Will Be Added With Space For Locating A Lift Enabling Access To The Ground Floor And First Floor By The Disabled. The Reconstruction Will Cover All Storeys Of The Building. New Partition Walls Will Be Built, New Door Openings Will Be Built In The Existing Internal Walls, Some Window Openings In The External Walls And Door Openings In The Internal Walls Will Be Bricked Up, Internal Load-Bearing Walls Will Be Partly Demolished, External Stairs Will Be Completely Dismantled And Rebuilt, Including One Of The Ventilation Chimneys Will Be Demolished, New Ventilation Chimneys Will Be Built. Flooring And Insulation Will Be Replaced. All Windows And Doors Will Be Replaced. Additional Insulation Of The External Walls Of The Above-Ground Storeys And Foundation Walls Will Be Made, Along With Damp-Proof Insulation, And The Roof Covering Over The Entire Building Will Be Changed. In The Basement Level, There Will Be Space For Technical Rooms, A Cleaning Room, A Server Room, A Medical Waste Room, A Gas Boiler Room As Well As Social Rooms For Employees With A Cloakroom With Sanitary Facilities. On The Ground Floor, It Is Planned To Create A Public Area For The Patient With A Waiting Room, Vaccination Point, Doctors Office, And Patient Registration. In Addition, It Is Planned To Create A Pharmacy Point With Facilities And W.C. For Patients, Children And Staff And A Cleaning Room. On The First Floor There Are: A Gynecological Office With Registration And Sanitary Facilities, A Treatment Room, A Dentists Office, An Ultrasound And Ecg Room, A Laboratory Collection Point, Two Doctors Offices, And A Toilet For Patients And A Cleaning Room. A Photovoltaic Installation With An Installed Capacity Of 9.9 Kwp Will Be Located On The Roof Of The Building. Scope Of Works For The Entire Construction Project And The Order Of Implementation: Scope Of Works: Demolition Works; Earthworks; Foundation Works; Construction Of Basement Walls; Execution Of The Walls Of The Ground Floor; Execution Of The Ceiling Above The Ground Floor; Execution Of The Walls Of The First Floor; Execution Of The Ceiling Above The First Floor; Execution Of Reconstruction; Execution Of The Roof Structure Together With The Covering; Execution Of The Facade; Execution Of Internal Installations; Finishing Works. Sequence Of Implementation: Demolition And Transfer Of The Fence; Demolition Of Existing Pavements; Execution Of Expansion And Reconstruction; Execution Of Gas Installation In The Ground; Access And Commuting, Greenery Arrangement. The Building Was Designed To Provide The Necessary Conditions For Use By The Disabled. The Subject Of The Contract And Quality Standards Are Detailed In The Design Documentation, Technical Specification For The Execution And Acceptance Of Works (Stwior) And The Bill Of Quantities. They Constitute An Integral Part Of This Swz. The Works Covered By The Subject Of The Order Should Be Performed In Accordance With The Design Documentation And The Conditions Contained In The Swz, The Contract Template And The Applicable Regulations, Standards And Principles Of Technical Knowledge And Construction Art, Due Diligence In Their Performance, Safety, Good Quality And Proper Organization. The Contracting Authority Requires A Site Inspection Within The Subject Of The Contract For Contractors Interested In Submitting A Tender. The Site Visit Is Intended To Familiarize The Contractor With The Specifics Of The Subject Of The Contract, Including The Conditions Of Its Execution. Each Of The Contractors Should Set The Date Of The Site Visit Individually With The Contact Persons Indicated In The Swz, Bearing In Mind That The Visits May Only Be Carried Out In The Presence Of The Employers Employees And On Dates Agreed In Advance. The Site Inspection May Take Place Only Before The Scheduled Date For Submission Of Tenders. People Who Come To The Site Inspection Are Required To Have An Identity Document With Them. A Report Signed By The Parties Will Be Drawn Up From The Site Visit, Which Will Constitute A Confirmation Of The Site Visit (A Template Of The Report Is Attached To The Tor). Of The Information Obtained And Other Consequences Of The Site Visit Are Borne By The Contractors Themselves. Failure To Participate In The Site Visit Of The Contractor Results In The Rejection Of His Offer Pursuant To Art. 226 Sec. 1 Point 18 Of The Public Procurement Law. All Materials Used Should Comply With Applicable Standards And Have The Required Attestations And Certificates, And Must Not Pose A Threat To The Hygiene And Health Of Users In Accordance With The Requirements Of The Act Of July 7, 1994 - Construction Law. Depending On The Materials Used, The Technology And Requirements Of The Manufacturers Must Be Strictly Observed. Before The Final Acceptance, A Set Of Pzh Certificates Should Be Presented And Attached To The Acceptance Documentation. The Contractor Will Also Include In The General Costs: 1) Comprehensive Geodetic Services, 3 Copies Of The As-Built Geodetic Inventory (Including 1 Copy On An Electronic Carrier). Vector File Format And Content. The File Format Is Dwg Or Dxf. 2) Preparation Of As-Built Documentation. 3) Obtaining A Positive Decision On The Occupancy Permit. Of The Contract. Extension Of Min. Guarantee And Warranty Period Is A Criterion For Evaluating Offers.
