1. The Subject Of The Contract Is The Execution, In The Design And Build Formula Based On The Functional And Utility Program (Pfu), Of The Investment, Including In Particular The Preparation Of Design Documentation And Construction Works For The Task Entitled: Improvement Of Water And Sewage Management In The Boćki Commune, Stage Ii. The Subject Of The Contract Consists Of The Following Tasks: 1/ Reconstruction Of The Water Supply Network From The Town Of Nurzec To The Town Of Mołoczki, 1900 M Long And Expansion In The Village Of Mołoczki, 150 M Long, 2/ Reconstruction And Expansion Of The Water Pumping Station In The Village Of Jakubowskie, Together With The Installation Of A Photovoltaic Installation With A Power Of 6Kw.3 / Reconstruction Of The Sewage Pumping Station In Boćki On Ul. Dubieńska, 4/ Reconstruction Of 4 Sewage Pumping Stations In Boćki And The Control System With Monitoring Of The Operation Of The Pumping Station (1 Pc. At 3-Go Maja Street, 2 Pcs. At Wojska Polskiego Street, 1 Pc. At Zarzecka Street), 5/ Construction Of A Sewage Treatment Plant At The Primary School In Andryjanki With A Capacity Of 1.6 - 2.0 M3/Day.2. The Subject Of The Contract Improvement Of Water And Sewage Management In The Boćki Commune, Stage Ii Is Implemented By The Boćki Commune As Part Of The Operation Entitled: Improvement Of Water And Sewage Management In The Boćki Commune, Stage Ii And Is Co-Financed By The European Union As Part Of The Operation Economy Water And Sewage System Under The Sub-Measure Support For Investments Related To The Creation, Improvement Or Expansion Of All Types Of Small Infrastructure, Including Investments In Renewable Energy And Energy Saving Covered By The Rural Development Program For 2014-2020, Aid Agreement Number: 00020-65150-Um1000157/20 Of March 31, 20213. The Subject Of The Contract Includes I.A. Design, Implementation Of Construction And Assembly Works, Disassembly And Removal Of Waste Generated, Start-Up Works Together With The Delivery Of Complete Health And Safety Equipment, Fire Protection, Training Of The Employers Staff.4. The Contracting Authority Informs That Offers Submitted In The Public Procurement Procedure Will Have To Cover The Entire Subject Of The Contract.5. The Detailed Scope Of The Contractors Obligations Is Specified In The Tender Documentation, In Particular In Appendix No. 2 To The Swz - Draft Provisions Of The Contract.6. A Detailed Description Of The Subject Of The Order And The Employers Requirements Regarding The Method Of Its Implementation Are Specified In The Order Documentation, In Particular The Functional And Utility Program.7. According To Art. 101 Sec. 4 Of The Public Procurement Law, If The Documents Describing The Subject Of The Contract Contain A Reference To The Standards, Technical Assessments, Technical Specifications And Technical Reference Systems Referred To In Art. 101 Sec. 1 Point 2 And Sec. 3 Of The Public Procurement Law And Such References Were Not Accompanied By The Phrase Or Equivalent, The Employer Allows Solutions Equivalent To Those Described In Each Such Standard, Technical Assessment, Technical Specification, Technical Reference System. In View Of The Above, It Should Be Assumed That Each: Standard, Technical Assessment, Technical Specification, Technical Reference System Appearing In The Description Of The Subject Of The Contract Is Accompanied By The Words Or Equivalent. Described In These Documents, Is Obliged To Prove, By Attaching To The Offer Relevant Evidence Referred To In Articles 104-107 Of The Public Procurement Law, That The Proposed Solutions Meet The Requirements Specified In The Description Of The Subject Of The Contract To An Equivalent Extent. Relevant Documents And Obtain The Employers Approval For Their Installation.8. Important Information Regarding The Subject Of The Contract: 8.1. Reconstruction Of The Existing Pvc 160 Water Supply System From The Village Of Nurzec To The Village Of Mołoczki, 1900 M Long And Extension Of The Water Supply Network By 150 M At The End Of The Village Of Mołoczki. 20 Years.The Water Supply System Is Located On A Wetland, Often There Are Failures That Are Difficult To Locate, At The End Of The Village There Are Plots That Cannot Be Connected To The Water Supply Network. The 1900 M Long Section Of The Water Supply Line Is Located In The Natura 2000 Area - Dolina Górnego Nurca Plb 200004 And Ostoja W Dolinie Górnego Nurca Plh 200021. The Existing Water Supply Line Runs Outside The Right-Of-Way Of The District Road Nurzec - Mołoczki.8.2. Water Pumping Station In Jakubowskie - Installation Of Two Water Equalization Tanks, 50 M3 Each, Replacement Of The Control Cabinet Of The Hydrophore Set Of The Water Pumping Station. The Existing Pump Set For Increasing The Pressure Is Built In The Village Of Jakubowskie On Plot No. 50/1 Gm. Storks In A Well Made Of Concrete Rings With A Diameter Of 250 Cm, Covered With A Reinforced Concrete Cover With An Entrance Hole With A Diameter Of 60 Cm. Minimum Water Pressure At The Inlet 0.10 Mpa, Minimum Water Pressure At The Outlet Of The Set 0.45 Mpa. Suction And Discharge Pipeline Of The Set Fi 1008.3. Reconstruction Of The Sewage Pumping Station At Ul. Dubieńska In Boćki - Replacement Of The Tank With Polymer Concrete With Equipment (Pumps, Check Valves, Gate Valves, Etc.) And A Control Cabinet. The Existing Pumping Station Is Built In A Right-Of-Way On A Municipal Road, Fenced. 8.4. Reconstruction Of 4 Sewage Pumping Stations In Boćki And The Control System With Monitoring Of The Operation Of The Pumping Station, I.E. Replacement Of Sewage Pumps In Four Sewage Pumping Stations And Replacement Of Four Control Cabinets Of The Sewage Pumping Station In Boćki. Control Cabinets Located At The Pumping Station Tank.8.5. Construction Of A Sewage Treatment Plant At The Primary School In Andryjanki With A Capacity Of 1.6 - 2.0 M3/D. 8.6. The Employer Stipulates That During The Works Being The Subject Of This Contract: The Water Supply From The Village Of Nurzec To The Village Of Mołoczki, The Sewage Pumping Station In Boćki, The Water Pumping Station In The Village Of Jakubowskie, Should Operate Continuously, Performing The Tasks For Which They Are Intended, And The Contractor Undertakes Perform The Subject Of The Contract In A Way That Ensures Uninterrupted Operation Of These Facilities And The Devices That Make Up Them.8.7. The Ordering Party Informs That As Part Of The Subject Of The Order, The Contractor Is Obliged To Obtain On His Behalf, In The Offer Price, All Studies, Permits And Administrative Decisions Necessary To Design, Perform And Hand Over The Planned Tasks For Operation And Fulfill All Obligations Imposed On The Ordering Party, Which Will Be Documented In The Above-Mentioned Documents.8.8. In Order For The Contractor To Obtain The Necessary Documents To Perform The Subject Of The Contract, The Ordering Party Shall Provide The Contractor With The Necessary Powers Of Attorney.8.9. In The Offer Price, The Contractor Is Obliged To Provide Geodetic And Cartographic Services Necessary To Complete And Accept The Subject Of The Contract And To Perform Water Tests (Regarding The Water Pumping Station In Jakubowskie And The Water Supply System In Mołoczki) And Treated Sewage (Regarding The Sewage Treatment Plant In Andryjanki).9. Requirements For The Works:9.1. Works Should Be Carried Out In Accordance With The Construction Art, With Applicable Laws, Technical And Technological Standards, With The Developed Design Documentation.10. Requirements For Materials, Construction Products And Devices:10.1. The Materials And Construction Products Used Must Be New, Of 1 Quality, Of High Quality And Of Documented Origin.10.2. All Materials, Devices, Machines And Equipment Used Must Have The Necessary Certificates, Approvals Or Technical Approvals, Operating Instructions Resulting From The Characteristics Of The Device. All Materials And Devices Together With The Necessary Documents (Attestations, Certificates, Etc.) Should Be Approved By The Investor Supervision Inspector (Inspector) Before The Planned Date Of Their Use/Assembly. All Devices Used Should Be Easy To Use And Have A Service In The Country. In Addition, Technological Equipment Cannot Be Prototype, Must Have Been Used In Other Facilities That Are The Subject Of The Contract, Have Appropriate National Certificates And Manufacturers Warranties, And Service Must Be Provided To Guarantee That Action Is Taken Within 48 Hours Of Reporting A Failure.10.3. Materials From Disassembly That Are Unsuitable For Reuse And Debris Must Be Disposed Of By The Contractor9 As Part Of The Remuneration Resulting From The Submitted Offer. The Cost Of Loading, Unloading And Transport To The Place Of Disposal Is Included In The Remuneration Resulting From The Submitted Offer.10.4. Materials From Disassembly Suitable For Reuse Should Be Deposited By The Contractor In The Place Indicated By The Employer. The Documents Contained In The Fup Constitute A Description Of The Subject Of The Contract In Accordance With Art. 103 Sec. 2 Of The Public Procurement Law Of September 11, 2019 (Consolidated Text, Journal Of Laws 2022, Item 1710, As Amended).
