Tenders Are Invited For Delivery Of A Set Of X-Ray Equipment To The Radiology Laboratory Of The Outpatient Clinic With The Sick Chamber Of The Remand Prison In Lublin

1. The Subject Of The Contract Is The Disassembly Of The Existing X-Ray Apparatus, Delivery, Assembly And Commissioning Of A New Set Of Digital X-Ray Apparatus In The Existing Room Of The X-Ray Laboratory Of The Outpatient Clinic With The Chamber Of Sick Remand Prison In Lublin.2. The Subject Of The Contract Includes, Among Others: 1) Disassembly Of The Existing X-Ray Equipment; 2) Delivery, Assembly, Installation, Start-Up Of Digital X-Ray Equipment With Accessories;3) Updating The Design Of Fixed Shields For Radiological Protection In Accordance With The Requirements Of The State Sanitary Inspector;4) The Set Of Digital X-Ray Equipment Offered By The Contractor Must Meet The Requirements Imposed By Law In Terms Of Admission To Market In The Territory Of The Republic Of Poland, Have Registration Certificates Required By Law In Accordance With The Act Of April 7, 2022 On Medical Devices (I.E. Journal Of Laws Of 2022, Item 974); The Results Of Which Will Be Attached To The Commissioning Report;6) The Employer Requires That The Full Warranty Period For The Set Of X-Ray Equipment Is At Least 24 Months, And All Costs Related To Assembly, Technical Inspections And Repairs, Including The Cost Of Spare Parts, Software Updates As Well As All Other Costs Related To Servicing, Including The Costs Of Travel, Accommodation And Labor, Are Included In The Purchase Cost; 7) The Contractor Must Provide The Necessary On-The-Job Training For Operating The X-Ray Equipment And Digital System Software For The Station Technician And Radiologist; 8) The Contractor Must Provide Service Provided By The Service Of The Manufacturer Of The Offered Device In Accordance With Applicable Law In This Regard;9) Software Updates, If It Is Necessary For The Proper Functioning Of The X-Ray Apparatus Set;10) All Installation Works Covered By The Subject Of The Contract Will Be Performed During The Operation Of The Outpatient Clinic , Therefore, The Works Must Be Performed In The Least Disruptive Way For Employees;11) In Order To Facilitate The Submission Of The Offer, The Employer Provides A Floor Plan Of The Room, Annex 2 To The Swz And Photos (1,2,3) Of The Electrical Board In The Radiological Laboratory - Annex No 3 To The Swz;12) Detailed Provisions Regarding The Implementation Of The Subject Of The Order Are Included In The Draft Provisions Of The Agreement, Which Is An Integral Part Of This Tor (Annex 4 To The Swz);13) Quantitative, Assortment And Detailed Parameters Of X-Ray Equipment Are Specified In The List Of Technical Parameters And Requirements For A Digital X-Ray Apparatus (Hereinafter: Wpt) - Constituting Appendix No. 1 To The Swz.
