We Are Issuing This Request For Proposal (Rfp) To Seek A Provider To Deliver Mental Health Services For The Members Of The Community.Kahkewistahaw First Nation Is Located 15Km North Of Broadview, Saskatchewan Along The #201 Highway, Next To The Qu&Rsquo;Appelle Valley. We Have A Population Of Approximately 2000+ Members, With Approximately 675 Living On Reserve.&Ldquo;The Mission Of Kahkewistahaw First Nation Health Centre Is To Enhance The Health Care Services Available Within The Community And Create Conditions Such That All People Living In The Community Have The Finest Holistic Health Care Available.&Rdquo;Purposethe Main Purpose Of Kahkewistahaw First Nation&Rsquo;S Mental Health Therapist Will Be To Provide The Highest Quality, Behavioral And Targeted Case Management Services To Individuals In Need. We Strive To Assist The Community&Rsquo;S Mental Health To Obtain The Services Needed And To Support Them In Their Goals To Improve Functionality And Their Integration In Community Settings, While Improving The Overall Quality Of Life. This Position Is Jointly Focused On Community-Based Programming And Client Referral Services. The Mental Health Therapist Will Be Under The Direct Supervision Of The Health Director.Vendors Shall Bear All Costs Associated With Preparing And Submitting Responses To This Rfp.Scope Of Work Develop And Implement A Community Based Mental Health Plan, Including Workshops, Public Meetings And Preparing Proposals For Funding.Assess, Screen, Prioritize And Provide Referrals To Appropriate Agencies.Provide Culturally Appropriate And Specific Individual, Family, And Group Therapy, With A Focus On Education And Prevention Of Addictions And Harmful Lifestyles.Establish And Maintain A Reporting System, Which Outlines Client Services And Utilizations, And Referrals.Submit Weekly/Monthly/Quarterly And Annual Activity Reports To The Health Director.Any Other Duties As Assigned By The Immediate Supervisor.Qualificationsplease Provide A Statement Giving A Brief History Of Your Work And Practice. Include A Description Of Why The Provision Of Mental Health Services With Kahkewistahaw First Nation Is Of Interest To You. Include A Copy Of Current License Or Certification To Provide Mental Health Services In The Province Of Saskatchewan.Referencesplease Provide Two To Four References Who Are Knowledgeable About The Vendor&Rsquo;S Skills And Commitment To Mental Health Service Delivery.Costs Of Servicesplease Include All Costs For Providing Mental Health Services To The Community.Additional Informationprovide Any Additional Information That Would Help Kahkewistahaw First Nation Evaluate The Proposal.
