4.1 The Subject Of The Contract Are Veterinary Services Provided As Part Of The Implementation Of The Program For The Care Of Homeless Animals And The Prevention Of Homeless Animals In The O ar w Mazowiecki Commune In 2023, Taking Into Account The Division Of The Contract Into Four Parts. 4.2 The Ordering Party Indicates The Territorial Scope Of Individual Parts Of The Order: 1) Part A Covers The Performance Of Veterinary Procedures From The Following Areas: - 1.Pilaszak w, 2.Pgr Pilaszk w, 3.Myszczyn, 4.Or y, 5.Pogroszew 6.Kolonia, .Umiast w, 9. Pgr Wolskie, 10. Wolskie, 11. Micha wek, 12. Koprki, 13. O tarzew, 14. O ar w Mazowiecki Precincts (0001, 0002, 0003), 4.3 The Implementation Of The Subject Of The Contract In The Scope Of Part A Includes The Performance Of: 1) Services In The Field Of Caring For Homeless Animals And Preventing Homeless Animals (Cats And Dogs) From The Area Of The O ar w Mazowiecki Commune; 2) Reimbursed Sterilization/Castration Treatments (Including Abortion Sterilizations) Of Owners Cats And Dogs, And Marking (Chipping) All Stray Dogs And Dogs Whose Owners Are Residents Of The O ar w Mazowiecki Commune; 3) Implementation Of The Subject Of The Order Includes The Performance Of The Following Veterinary Procedures: - Sterilization/Castration (Including Abortion Sterilization), - Euthanasia Of Blind Litters, - Inpatient Treatment Including Medical Activities, - Euthanasia, - Checking The Chip Number Each Time, - Stay At The Clinic Including Keeping The Animal, - Flea And Deworming, - Fiv/Felv Tests - In The Case Of Cats, - X-Ray Examination, - Ultrasound Examination, - Morphology - Blood Test, - Faecal Examination, - Rabies Vaccination, - Vaccination Against Infectious Diseases At The Same Time, That The Place Of Providing Veterinary Services May Be Located At A Maximum Distance Of 5 Km From The Administrative Borders Of The O ar w Mazowiecki Commune, And This Distance Should Be Measured Along The Shortest Route That The Vehicle Will Travel On Public Roads Between The Address Indicated As The Place Of Providing Services And The Border Of The Commune Ozarow Mazowiecki. The Contractor Shall Submit A Relevant Declaration In The Tender Form To That Effect. 4.8 The Subject Of The Order For Part A Should Be Implemented In Accordance With The Description Of The Subject Of The Order According To Appendix No. 1 To Swz.4.9 The Task Is Included In The Communes Budget For 2023 In Section 900, Chapter 90095 4300.
