The Subject Of The Order Is The Supply Of Srs Urogynecological Implants For The Needs Of The Mi dzyleski Specialist Hospital In Warsaw, In The Assortment And Quantities Specified In Detail In The Price Form Constituting An Integral Part Of This Swz.2. The Ordering Party Reserves The Right, And The Contractor Agrees, To Place Orders Without Restrictions Under This Agreement, As Well As The Right To Adjust The Ordered Quantities Downwards Or Upwards In The Event Of Circumstances That The Ordering Party Could Not Have Foreseen, With The Proviso That The Minimum Value Of The Entire Order Will Not Be Less Than 30%.3. The Ordering Party Does Not Allow Variant Offers. 4. Each Contractor Has The Right To Submit Only One Offer. Submission By One Contractor Of More Than One Offer Or An Offer Containing Alternatives Results In The Rejection Of All Offers Submitted By A Given Contractor.5. The Assortment Offered In The Tender Procedure (Listed In The Price Form) Must Be Consistent With The Description Of The Subject Of The Contract And Must Meet The Conditions Of Admission To Trading In Health Care Facilities In Accordance With Applicable Law.6. Failure To Meet Any Of The Required Parameters Gives The Ordering Party Grounds To Reject The Offer.7. The Delivery Time Cannot Be Longer Than 7 Days.8. Guarantee Period Minimum 24 Months9. The Contract Will Be Performed For A Period Of 12 Months From The Date Of Concluding A Valid Contract With The Contractor Selected As A Result Of The Public Procurement Procedure. 10. Offers Will Be Evaluated On The Basis Of The Criteria Described In Chapter Xv Of This Swz.11. The Draft Provisions Of The Agreement Constitute Appendix No. 1 To The Swz, Which Is An Integral Part Of This Documentation. The Provisions Contained In The Swz Should Be Treated As Conditions For Awarding The Contract12. Subcontracting1. The Contractor May Entrust The Performance Of Part Of The Contract To Subcontractors,2. The Contractor Is Obliged To Indicate In The Offer The Parts Of The Contract, The Performance Of Which He Intends To Entrust To Subcontractors, Specifying The Companies Of Subcontractors By The Contractor.3. The Contracting Authority Informs That It Does Not Reserve The Contractors Personal Performance Of Key Parts Of The Contract.4. If The Ordering Party Finds That There Are Grounds For Exclusion With Respect To A Given Subcontractor, The Contractor Is Obliged To Replace This Subcontractor Or Resign From Entrusting The Performance Of A Part Of The Contract To A Subcontractor.5. Entrusting The Execution Of Part Of The Order To Subcontractors Does Not Release The Contractor From Responsibility For The Proper Performance Of This Order.
