1. The Subject Of The Contract Is The Provision Of Veterinary Services For Animals From The Municipality Of The City Of Tomasz w Mazowiecki.2. The Scope Of The Order Includes Stationary (In A Veterinary Office Located In The Municipality Of The City Of Tomasz w Mazowiecki) And Away (In The Field During Chipping Campaigns Carried Out Periodically In Housing Estates And During Cultural Events In The Municipality Of Miasto Tomasz w Mazowiecki) Microchipping And Vaccination Against Rabies Of Owner Dogs And Cats, Whose The Owners Live In The Municipality Of Tomasz w Mazowiecki.3. The Forecasted Number Of Treatments Will Be 2,000 Chipping Procedures And 2,000 Vaccinations Against Rabies Of Owners Dogs And Cats From The Municipality Of The City Of Tomasz w Mazowiecki. 4. The Projected Maximum Number Of Away Chipping Actions Will Be 15.5. The Number Of Treatments Given Above Is Indicative. It May Be Reduced, Depending On The Actual Level Of Interest Of Persons Entitled To Use Veterinary Services Covered By The Subject Of The Contract. It Is Possible To Change The Number Of Individual Services Indicated Above While Maintaining The Unit Prices Specified In The Offer. These Changes Cannot Increase The Total Value Of The Order. The Minimum Number Of Performed Chipping Procedures Being The Subject Of The Order Will Not Be Less Than 40% Of The Value Indicated In Section 3, I.E. 800 Treatments.6. The Detailed Scope Of The Subject Of The Contract Is Specified In Part Iv Of The Swz - Constituting Its Attachment (Separately For Each Part Of The Contract).
