1. The Subject Of The Contract Is Multi-Branch Investor Supervision Over Construction Works Performed As Part Of The Task Modernization Of The Seat Of The District Office At Ul. Młynarska 16 For The Needs Of The Registry Office In The Following Specialties: A) Construction And Construction, B) Installation In The Field Of Heating, Ventilation, Water Supply And Sewage Installations And Devices, C) Installation In The Field Of Electrical Installations And Devices, D) Installation In The Field Of Installation And Telecommunications Equipment. 2. The Ordering Party Will Entrust, As Part Of The Execution Of This Order, Exercising Investors Supervision Over The Implementation Of The Task In Order To Effectively Enforce From The Contractor Of Construction Works The Requirements Regarding Timeliness, Correct Execution And Quality Of Works, Materials Used, Acceptance Of Works, Coordination Of All Activities Related To The Implementation Tasks Until The Final Occupancy Permit Is Obtained. The Following Elements: 1) Demolition, Demolition And Disassembly Works - Demolition Of Parts Of Partition Walls; 2) Masonry Works - Construction Of New Partition Walls, Bricking Up Fragments Of Door Openings; 3) Assembly Of A Passenger Lift Adapted For Disabled People In Wheelchairs; 4) Replacement Of Parts Window And Door Joinery In Accordance With The List Of Joinery; 5) Replacement Of Sanitary And Electrical Installations; 6) Finishing Works; 7) Reconstruction And Adaptation Of Internal Installations To The New Arrangement Of Rooms In Accordance With The Technical Design. 4. The Contractor Will Be Obliged To Supervise Construction Works, The Detailed Scope Of Which Is Defined In The Design Documentation Prepared By Mm Proj -Bud Marcin Młodyankiewicz, Ul. Gen. Józefa Hallera 65A/3, 87-100 Toruń, Which Constitutes Appendix No. 11 To Swz.5. Facility Characteristics: The Project Consists In The Reconstruction Of The Building Of The District Office. The Building Is Located On Plot No. 51 In The Area Of ​​0316 Warszawa At Ul. Młynarska 16. The Building Is Located At The Northern Border Of The Plot. Communication Service Is Provided By Ul. Młynarska From The Western Side Of The Plot. The Scope Of The Planned Works Covers Only The Internal Part Of The Building. The Reconstruction Of The Building Is To Improve The Functionality Of The Building And Be Adapted To The Needs Of The Registry Office. On The Basement Level Of The Building, There Are Archive Rooms And Rooms With A Technical Function: A Heat Distribution Center And Horizontal And Vertical Communication. On The Ground Floor And First Floor There Are Rooms Where The Function Of The Registry Office Will Be Performed. On The Ground Floor There Is A Public Area Consisting Of A Vestibule, Communication, A Customer Service Office And A Sanitary Facility For Visitors And Staff, As Well As A Limited Access Zone Consisting Of Office Rooms. On The First Floor, A Wedding Hall, An Office Room, A Sanitary And Hygienic Facility, Cloakrooms For Visitors, As Well As Horizontal And Vertical Communication Are Planned. On The Second Floor, Only The Staircase Will Be Separated With A Wall And A Door To Separate The Registry Office From The Rooms Of The Wola District Office. The Architectural Form Of The Building Will Not Change. Currently, It Is Adapted To The Landscape And Surrounding Buildings. Information On The Division / Lack Of Division Of The Order Into Parts. Divided The Contract Related To The Supervision Service Into Parts, Each Of Which Is The Subject Of Separate Proceedings. This Proceeding Concerns One Of The Above-Mentioned Part Of The Order. 214 Sec. 1 Point 7 Of The Public Procurement Law. 12. Pursuant To § 3 Sec. 6 Of The Contract Template, The Contractor Is Obliged To Have A Contract Civil Liability Policy For Professional Activity In The Scope Covering The Subject Of The Contract Referred To In §1 Of The Contract, For The Amount Of At Least Pln 100,000.00 (In Words: One Hundred Thousand Zlotys), Throughout The Entire Duration Of The Contract. 9. Subcontractors: All Regulations And Requirements Regarding Subcontracting Have Been Included By The Ordering Party In The Draft Provisions Of The Contract - The Contract Template, Constituting Annex No. 7 To The Swz.
