1. The Subject Of The Contract Is The Execution Of Construction Works As Part Of The Task Entitled Modernization Of The Seat Of The District Office At Ul. Młynarska 16 For The Needs Of The Registry Office. 2. The Subject Of The Contract Includes The Performance Of Construction Works On The Basis Of The Design Documentation Listed In Point 5.1), Which Consist In Performing, Among Others, The Following Elements: A) Demolition, Demolition And Disassembly Works - Demolition Of Parts Of Partition Walls, B) Masonry Works - Construction Of New Partition Walls, Bricking Up Fragments Of Door Openings, C) Assembly Of A Passenger Lift Adapted For Disabled People In Wheelchairs, D) Replacement Of Parts Window And Door Joinery In Accordance With The List Of Joinery, E) Replacement Of Sanitary And Electrical Installations, F) Installation Of Ventilation, F) Finishing Works, G) Reconstruction And Adaptation Of Internal Installations To The New Arrangement Of Rooms In Accordance With The Technical Design, 3. Characteristics Of The Facility. The Project Consists In The Reconstruction Of The Building Of The District Office. The Building Is Located On Plot No. 51 In The Area Of 0316 Warszawa At Ul. Młynarska 16. The Building Is Located At The Northern Border Of The Plot. The Communication Service Is Provided By Ul. Młynarska From The Western Side Of The Plot. The Scope Of The Planned Works Covers Only The Internal Part Of The Building. The Reconstruction Of The Building Is To Improve The Functionality Of The Building And Adapt It To The Needs Of The Registry Office. On The Basement Level Of The Building, There Are Archive Rooms And Rooms With A Technical Function: A Heat Distribution Center And Horizontal And Vertical Communication. On The Ground Floor And First Floor There Are Rooms Where The Function Of The Registry Office Will Be Performed. On The Ground Floor There Is A Public Area Consisting Of A Vestibule, Communication, A Customer Service Office And A Sanitary Facility For Visitors And Staff, As Well As A Limited Access Zone Consisting Of Office Rooms. On The First Floor There Will Be Rooms For The Wedding Hall, Offices, Sanitary And Hygienic Facilities, Cloakrooms For Visitors, As Well As Horizontal And Vertical Communication. On The Second Floor, There Will Only Be A Separation Of The Staircase With A Wall And A Door To Separate The Registry Office From The Premises Of The Employment Office. The Architectural Form Of The Building Will Not Change. The Surroundings Of The Building (External Area) Are Not Included In The Scope Of The Study. 4. Construction Works Will Be Carried Out In An Active Facility, Which Is A Public Utility Building. The Implementation Of The Subject Of The Contract Should Provide For The Construction Works To Be Carried Out In Stages On The Basis Of The Design Documentation Referred To In Point 5.1), Caused By The Fact That Construction Works Will Be Carried Out On An Active Facility. The Organization Of Works Must Take Into Account The Specificity Of The Facility, The Consequent Limitations And Must Not Interfere With The Operation Of The Facility. Prior To The Commencement Of Works, The Ordering Party And The User (Director Of The Facility) Should Be Presented With A Work Organization Plan Ensuring The Least Nuisance For The Facility For Approval.5. A Detailed Description Of The Subject Of The Order, The Scope Of Work And The Requirements Regarding The Execution And Acceptance Of The Subject Of The Order Are Specified In: “Modernization Of The Seat Of The District Office At Ul. Młynarska 16 For The Needs Of The Registry Office - Reconstruction Of A Part Of The Building Of The District Office For The Needs Of The Civil Registry Office At Ul. Młynarska 16 In Warsaw, Developed By Pracownia Projektowa Mm Proj-Bud Marcin Młodyankiewicz, 87-100 Toruń, Ul. Gen. Józefa Hallera 65A/3, Which Includes The Following Documents: A. Land Development Plan, B. Architectural And Construction Design, C. Technical Projects In The Construction, Electrical And Sanitary Industries, D. Technical Specifications For The Execution And Acceptance Of Construction Works In The Construction, Electrical And Sanitary Industries, E. Bills Of Quantities For Information/Auxiliary Purposes, F. Decision On The Building Permit No. 16/N/2023 Of February 22, 2023, G. Fire Protection Expertise Document On The Criteria Used To Assess Equivalence5.2) Draft Provisions Of The Contract, Hereinafter Also Referred To As The Contract Template - Constituting Annex No. 6 To Swz.6. It Is Recommended That The Contractor Familiarize Himself With The Design Documentation Listed In Point 5.1 Before Submitting The Offer. 7. In The Proceedings In Question, The Contracting Authority Applies The Social Clause Pursuant To Art. 95 Of The Public Procurement Law. The Employer Requires That Persons Performing The Subject Of The Contract And Performing Activities Related To Construction, Electrical And Sanitary Works Be Employed By The Contractor Or Subcontractor, On The Basis Of An Employment Relationship, In Accordance With The Act Of June 26, 1974. Labor Code (Journal Of Laws Of 2022 Item 1510 As Amended). Detailed Requirements Regarding The Method Of Documenting And Verifying The Employment Of These Persons And The Employers Rights To Control This Employment And Sanctions For Failure To Meet These Requirements Are Included In The Contract Template, Constituting Appendix 6 To Swz.8. Information On The Division / Lack Of Division Of The Contract Into Parts. The Contracting Authority Did Not Divide This Contract Into Parts, As Such A Division Would Result In Excessive Technical Difficulties And Excessive Costs Of Contract Performance. The Need To Coordinate The Activities Of Various Contractors Performing Individual Parts Of The Contract Could Seriously Jeopardize The Proper Performance Of The Contract. 9. Pursuant To § 3 Sec. 2 Of The Contract Template, The Contractor Is Obliged To Have A Civil Liability Policy For The Conducted Activity For The Sum Insured Not Less Than Pln 5,000,000.00 (In Words: Five Million Pln 00/100), Throughout The Duration Of The Contract, Which Will Cover His Liability Both Contractual And Tort. 10. The Ordering Party Informs That It Intends To Allocate The Gross Amount Of Pln 5,854,127.33 To Finance The Awarded Contract.11. Guarantee And Warranty: The Contractor Will Provide A Guarantee For The Completed Subject Of The Contract For A Minimum Period Of 36 Months. The Contractor May Propose To Extend The Warranty Period For The Completed Subject Of The Contract To 60 Months. The Warranty Period For The Completed Subject Of The Contract Is A Criterion For Evaluating Tenders - Chapter Xiv Swz. The Warranty Period Is 60 Months - § 11 Section 3 Of The Contract Template.12. The Ordering Party Does Not Require A Site Visit.13. The Ordering Party Does Not Provide For The Possibility Of Awarding Contracts Pursuant To Art. 214 Sec. 1 Point 7 Of The Public Procurement Law. 14. Subcontractors: All Regulations And Requirements Regarding Subcontracting Have Been Included By The Ordering Party In The Contract Template, Constituting Appendix No. 6 To The Swz.
