1. The Subject Of The Contract Awarded Is The Collection Of Waste In The Form Of Plastic Bottles And Aluminum Cans From Two Bottle Machines Located In Park Im. E. Szymański And At The Kindergarten On Ul. Boguszewska Together With Their Ongoing Service For A Period Of 8 Months.2. A Detailed Description Of The Subject Of The Order, The Type And Scope Of Works Covered By The Order And The Requirements Regarding The Execution And Acceptance Of The Subject Of The Order Are Specified In The Draft Provisions Of The Contract - The Contract Template - Constituting Appendix 2 To Swz.3. In The Proceedings In Question, The Contracting Authority Applies The Social Clause Pursuant To Art. 95 Of The Public Procurement Law. The Employer Requires That Persons Performing The Subject Of The Contract And Performing Activities Consisting In Emptying The Bottling Machines Are Employed By The Contractor Or Subcontractor, On The Basis Of An Employment Relationship, In Accordance With The Act Of June 26, 1974 - Labor Code (I.E. Journal Of Laws Of 2022, Item 1510 With Amendments). Detailed Requirements Regarding The Method Of Documenting And Verifying The Employment Of These Persons And The Employers Rights To Control This Employment And Sanctions For Non-Compliance With These Requirements Are Contained In The Contract Template, Constituting Appendix 2 To Swz.4. Information On The Division/Lack Of Division Of The Contract Into Lots. This Proceeding Concerns One Of The Above-Mentioned Parts Of The Order.5. Pursuant To § 3 Sec. 5 Of The Contract Template, The Contractor Is Obliged To Have A Civil Liability Policy For The Conducted Activity For The Sum Insured Not Less Than Pln 100,000.00 (In Words: One Hundred Thousand Zlotys 00/100), For The Entire Duration Of The Contract.6. The Ordering Party Informs That It Intends To Allocate The Gross Amount Of Pln 183,600.00 To Finance The Awarded Contract.
