1. The Subject Of The Order Is The Supply Of Professional Disinfectants Intended For Use In Medical Offices In The Facilities Of Uzdrowisko Kołobrzeg S.A. In Accordance With Appendix No. 1.2. A Detailed Description Of The Chemicals Is Set Out In Appendix 1 To This Specification.3. Disinfectants Are Delivered By The Contractor.4. Deliveries According To Monthly Needs,5. Deliveries At The Cost Of The Supplier To The Indicated Place,6. Payment By Bank Transfer 30 Days From Issuing The Invoice7. All Offered Preparations Must Be Intended For Use In The Medical Area, I.E. In Doctors Offices, Nursing Treatment Rooms, Isolation Rooms, Treatment Base Rooms.8. The Disinfectants Offered Should: A) Meet The Requirements Of The Law, B) Be Of High Quality And Fulfill The Function For Which They Were Intended, C) Be Registered Medical Devices With A Broad Spectrum Of Activity, C) Have A Shelf Life Of Not Less Than 12 Months, Counting From Delivery Date, D) Do Not Have Mutagenic And Carcinogenic Properties, E) Should Not Damage The Surfaces To Which They Will Be Applied.9. In Order To Confirm That The Offered Disinfectants Meet The Requirements Specified By The Ordering Party, The Ordering Party Requests That The Following Documents Be Attached To The Offer: - That Each Disinfectant Has An Entry In The Register Of Medical Devices Or A Declaration Of Conformity, - That Each Offered Disinfectant Has Valid Permanent Permits For Use And Issued By The Minister Of Health, - Copies Of Documentation Certified As True Copies By Authorized Persons - Statements, Clinical Trials, Certificates, Marketing Authorizations Prepared In Foreign Languages ​​Should Be Submitted Together With A Translation Into Polish Prepared By Authorized Persons And At The Contractors Expense. - The Disinfectants Offered Must Meet The Following Requirements, Depending On The Type Of Disinfectant: • Act Of 6 September 2001. An Offer Containing Documents Confirming That The Tests Performed, Meaning That The Subject Of The Order Has Been Tested On The Test Organisms Listed Below And Meets The European Standards For The Medical Area (Standards, At Least Phase Ii) Or Polish Standards For The Medical Area (Standards, At Least Phase Ii ) Or Has Been Tested At The National Institute Of Public Health - National Institute Of Hygiene: 1) Bactericidal (B); 2) Mycobactericidal (Tbc) - Mycobacterium Tuberculosis Or Terre And Avium; 3) Fungicidal (F); 4) Sporicidal (S) - Bacillus Subtilis, Bacillus Cereus, Clostridium Sporogenes;5) Virucidal (V) 11. If The Offered Agent Is On The List Of The National Institute Of Public Health - National Institute Of Hygiene, It Is Not Required To Attach Confirmations Of The Tests Performed.12. The Offer Should Also Be Accompanied By Current Or Updated In The Last 2 Years Safety Data Sheets For Each Disinfectant. 13. Documents Concerning Only The Offered Products Should Be Attached To The Offer.14. In The Event Of Changes Resulting In Changes To The Documents Attached To The Offer, The Supplier Shall Immediately Deliver All Documents To The Recipient.15. During The Term Of The Contract, The Supplier Shall Immediately Provide The Ordering Party With Changes In The Documentation Of The Delivered Disinfectant Regarding Withdrawal, Limitation Of Use / Application, Significant Changes In The Results Of Clinical Trials, Etc.16. On The Day Of The First Delivery, The Contractor Is Obliged To Deliver Free Of Charge Sprinklers, Measuring Cups/Pumps/Dosing Valves, Taps For Pouring Preparations.17. The Funds Must Be Delivered At The Suppliers Expense In Original Packaging, Without Damage. Substitute Labels Are Not Allowed. The Following Must Be Visible On The Packaging: - Name Of The Product, Expiry Date, Batch Number, Quantity/Weight Of The Packaging, Manufacturer, Occupational Safety Measures, - Inscriptions On Collective And Unit Packaging Must Have Labels In Polish. 18. The Supplier Undertakes To Collect The Packaging Of The Delivered Disinfectants.19. The Ordering Party Requires The Execution Of The Order Within No More Than 36 Hours From The Moment Of Placing The Order, Excluding Non-Working Days (Saturday, Sunday, Holidays).20. The Supplier Is Obliged To Deliver The Preparations To Individual Facilities Of The Health Resort At The Indicated Address, According To The Appendix To The Contract.21. Delivered Funds Must Be Packaged In A Way That Protects Them From Damage22. The Ordering Party Reserves The Right To Reduce The Number Of Assortment Items Purchased Under This Agreement, Listed In The Offer, Depending On The Current Needs Of The Ordering Party.23. Reducing The Quantity Of The Subject Of The Contract Purchased (E.G. Due To Lower Consumption) Will Not Result In Any Legal And Financial Consequences For The Ordering Party.24. . The Supplier Is Obliged To Deliver The Ordered Disinfectant Listed In The Appendix To The Offer.25. If The Agent Specified In The Agreement Is Not Available, The Supplier, Upon Agreement And With The Consent Of The Ordering Party, Is Obliged To Provide The Ordering Party With A Disinfectant With Identical Properties And The Same Spectrum Of Action And At The Price Of The Agent Specified In The Agreement.26. The Supplier Is Obliged To Provide The Ordering Party With Disinfectants In Accordance With The Agreement.27. In The Event Of Failure To Comply With The Terms Of The Contract, The Ordering Party Shall Purchase Funds From Another Supplier, Charging The Costs Indicated In The Contract .28. The Ordering Party Reserves The Right To Deliver Funds Other Than Those Indicated In The Appendix To The Contract.29. The Supplier Will Conduct Training In Occupational Health And Safety Using The Supplied Disinfectants For Employees Of Uzdrowisko Kołobrzeg S.A. Using Funds.30. Deliveries Will Be Made Successively In Batches In Accordance With Partial Orders Specifying The Type And Quantity Of Disinfectants By Authorized Persons.
