Work Detail
3.1. The Subject Of The Contract Is The Extension, Superstructure And Reconstruction Of Five Buildings, Including The Demolition And Change Of Use To Office Buildings Of The Municipal Office In Kamieńsk, Including: Task 1 - Comprehensive Modernization Of The Building Of The Municipal Office In Kamieńsk, Including: 1) Reconstruction And Superstructure Of The Office Building No. 1; 2) Reconstruction Of Office Building No. 2; 3) Reconstruction, Extension And Superstructure Of Building No. 4 Along With A Change Of Use To An Office Building; 4) Reconstruction Of Building No. 5 With A Change Of Use To An Office Building; 5) Construction Of An External Sanitary Sewage System; 6) Installation Of A Photovoltaic Installation. As Part Of This Task, Thermal Modernization Of Buildings, Internal And External Renovation And Modernization Works Will Be Carried Out, And A Photovoltaic Installation Will Be Installed. Task 2 - Adaptation Of The Existing Garages For A Meeting Room, Integration And Social Activation In Kamieńsk. The Scope Of The Task Includes The Demolition And Reconstruction Of Building No. 3, Including The Change Of Use To An Office Building (The Scope Of This Task Ends At The Junction Of The Meeting Room (Room Number 1/22 In Figure B-3 Of The Architectural And Construction Design) With The Corridor (Room Number 1/19 In Drawing B-3 Of The Architectural And Construction Design) And Includes The Insertion Of Doors Along With Two-Sided Flashing Between These Rooms. The Newly Created Room Will Accommodate Up To 50 People. It Is Planned To Demolish The Garage (External And Internal Walls And Roofing) And Build New Structural And Partition Walls, A Roof Structure In The Form Of Wooden Trusses Covered With Trapezoidal Sheet Metal.The Floor Level Will Be Leveled To The Level Of The Internal Corridor Of The Central Part Of The Building. Entrance, Where New Stairs Made Of Concrete Cubes With A Profiled Driveway For The Disabled Are Planned. There Are Toilets And A Cloakroom Right At The Entrance. The North-Western Part Of Building No. 3 Is Single-Storey. In This Part There Is A Utility Room, A Boiler Room And A Disused Chute That Will Be Demolished. The Building Is Located In Kamieńsk, Commune Kamieńsk, Plot No. 119/1, Precinct 4. Category Of The Building: Xii - Public Administration Buildings. Note: The Contract Will Be Performed On An Open Facility. The Contractor Should Plan The Works In Such A Way As To Ensure The Proper Functioning Of The Municipal Office In Kamieńsk. The Detailed Description Of The Subject Of The Contract For Both Tasks Is - Architectural And Construction Design, Technical Designs, Stwiorb, Bills Of Quantities And Additional Explanations Regarding The Description Of The Subject Of The Contract. Note: Due To The Adopted Nature Of The Contractors Remuneration (Lump Sum), The Bills Of Quantities For The Works Are Only Informative.3.2. Acting On The Basis Of The Content Of Art. 310 Of The Public Procurement Law, The Contracting Authority Reserves The Right To Cancel The Contract Award Procedure If The Public Funds Which The Contracting Authority Intended To Allocate To Finance The Whole Or Part Of The Contract Have Not Been Granted.3.3. The Employer Requires That Activities In The Scope Of Performance Of Works In The Construction And Construction Industry, The Sanitary Industry And The Electrical Industry Be Performed By Persons Employed By The Contractor, Subcontractor On The Basis Of An Employment Relationship Within The Meaning Of Art. 22 § 1 Of The Act Of June 26, 1974. - Labor Code (I.E.: Journal Of Laws Of 2022, Item 1510 With Amendments). The Persons Mentioned In The First Sentence May Not Perform Any Activities On The Construction Site Without Employment Under An Employment Relationship With The Contractor, Subcontractors Of Works Or Services. This Requirement Does Not Apply To The Construction Manager, Works Managers, Suppliers Of Construction Materials, Surveyors, Persons Providing Equipment And Transport Services, Unless These Activities Constitute An Employment Relationship Under The Provisions Of The Labor Code (E.G. Self-Employment).3.4. The Contracting Authority Informs That Where The Subject Of The Contract Is Described In The Swz By Indicating Trademarks, Patents Or Origin, Source Or Specific Process That Characterizes The Products Or Services Provided By A Specific Contractor, It Allows Solutions Equivalent To Those Described, Provided That They Meet The Requirements Specified In The Description Of The Subject Of The Contract Criteria Used To Assess Equivalence. Where The Ordering Party Has Described The Subject Of The Contract By Reference To Standards, European Technical Assessments, Technical Specifications And Technical Reference Systems Referred To In Art. 101 Sec. 1 Point 2 And Sec. 3 Of The Ppl, The Employer Allows Solutions Equivalent To Those Described, And Such A Reference Is Accompanied By The Words Or Equivalent. The Employer Indicates For These Solutions As The Criteria Of Equivalence Allowing A Tolerance Of +/- 5% For All Parameters Required For Them. The Employer Indicates That The Obligation To Submit Solutions Equivalent To To Those Described In The Description Of The Subject Of The Contract And To Demonstrate Equivalence Lies With The Contractor. If The Requirements In The Description Of The Subject Of The Contract Refer To A Trademark, Patent Or Origin, Source Or Specific Process That Characterizes The Products Or Services Provided By A Specific Contractor, The Contracting Authority Requires That The Contractor, In The Case Of Offering Equivalent Solutions, Already Present In Its Tender An Evidence Of Equivalence Confirming The Fulfillment Of The Criteria Specified In The Description Of The Subject Of The Contract, Which The Ordering Party Uses To Assess Equivalence (Cjeu Judgment C-14/17 Of 2018-07-12), E.G. Product Data Sheets. Where The Requirements In The Description Of The Subject Of The Contract Refer To European Technical Assessments, Technical Specifications And Technical Reference Systems Referred To In Art. 101 Sec. 1 Point 2 And Sec. 3 Of The Public Procurement Law, The Contracting Authority Requires That The Contractor Has Already Presented Evidence Of Equivalence In Its Tender In The Manner Specified In Art. 101 Sec. 5 And 6 Pzp.3.5. Prior To The Commencement Of Works, The Contractor Is Obliged To: • Mark The Construction Site (Information And Warning Boards), • Notify The Owners Of The Neighboring Plots And Any Persons Staying On The Neighboring Plots About The Works Being Carried Out, 3.6. Supplementary Works Required By The Employer: 1) Performing Full Geodetic Service Of The Investment (As Of The Acceptance Date, The Contractor Should Have An As-Built Geodetic Map With Stamps Confirming That It Was Entered Into The Geodetic Resource By The Center For Cartographic And Geodetic Documentation), 2) Preparation Of Acceptance Documentation In Two Copies In Polish, Broken Down By Industries (Bound In Binders With A Detailed Table Of Contents) Containing At Least: A) Permission To Use Or Application For Use To The Pinb Accepted Without Objection, B) Original / Photocopy Of The Construction Log, C) As-Built Geodetic Maps With Stamps Confirming That They Were Entered Into The Geodetic Resource By The Documentation Center Cartographic And Surveying, D) The Original Of The Site Managers Written Statement: • On The Execution Of Works In Accordance With The Construction Design, The Final Decision Of The Building Permit And The Construction Law, • On Bringing The Construction Site To A Proper Condition And Order, As Well As Roads, Streets, Neighboring Properties, Buildings And/Or Premises, • On The Appropriate Development Of The Areas Adjacent To The Construction Site Enabling Proper Operation Of The Constructed Facility. E) List Of Built-In Materials Along With Documents Confirming Their Admission To Use In The Republic Of Poland, G) List Of Significant Changes (If Any) Place) With A Graphic Attachment, Signed By The Site Manager, The Works Manager Appropriate To The Characteristics Of The Industry Affected By The Change, The Supervision Inspector, The Designer, Along With Documents Confirming The Correct Legalization Of These Changes, H) Test Reports Performed During Construction Works, I) Electronic Version (In The Amount In Accordance With The Paper Version) Acceptance Documents And As-Built Documentation (Inventory In Pdf Format) On A Cd/Dvd. The Task Is Co-Financed By The Governmental Fund Polski Ładprogram Inwestycji Strategicnych.
