I. The Subject Of The Contract Is The Provision Of Sanitary Transport Services For Polkowice Centrum Usług Zdrowotnych - Zoz S.A., Mainly In The Dolnośląskie Voivodeship, In Particular The Commune Of Polkowice, Lubin, Głogów. Ii. Detailed Description And Method Of Order Fulfillment: 1. In The Scope Of The Provision Of Sanitary Transport Services: 1) Services Will Be Provided From Monday - Friday From 18.00 - 6:00, Saturdays, Sundays And Holidays 24 Hours A Day. In The Field Of Transport For The Needs Of Zol (Zakład Opiekuńczo-Healing Centre) From Monday To Friday From 8.00 To 15.00 2) Scope Of Tasks Performed By Sanitary Transport: A) Transport Of Nurses, Doctors, Employees, With Or To The Patient, B) Transport Of Medical Equipment C) Transport Of The Patient To Primary Health Care Facilities, Hospital Or To A Place Indicated By A Doctor, D) Transport Of Zol Patients For Consultation Or Diagnostic Tests In Neighboring Hospitals, 3) The Contractor Is Obliged To Keep Statistical Documentation, In Accordance With Applicable Law. Services Will Be Provided On The Basis Of Non-Cash Settlements Based On Completed Orders And Transport Cards, On Which The Employers Employee Will Confirm The Performance Of The Transport Service On An Ongoing Basis. The Contractor Transfers Payment For The Service On The Basis Of A List Of Transports Provided In The Month Preceding The Issue Of The Invoice, Together With Transport Orders/Cards Constituting The Basis For The Settlement. The Contractor Shall Provide The Ordering Party With The Transport Statement In Writing. 4) The Ordering Party Requires That The Contractor Has Valid Insurance Contracts, Including Third Party Liability Insurance, Ac, Accident Insurance And Valid Technical Inspections Of The Vehicle, Throughout The Entire Period Of The Contract. In A Situation Where The Insurance Contract Expires During The Term Of The Contract, The Contractor Shall Be Obliged To Conclude A New Insurance Contract On Time To Ensure The Continuity Of Performance Of The Contract In Question. The Value And Material Scope Of The Insurance Contract Must Comply With Applicable Law. Ensuring The Performance Of Transport In Accordance With The Conditions Referred To In Chapter 8 Of Regulation No. 77/2015/Dsoz Of The President Of The National Health Fund Of November 19, 2015.6) Waiting Time For Passenger Transport Not Longer Than 15 Minutes. During The Covid-19 Epidemic, No Longer Than 60 Min. ) Was On Standby, Which Means Accepting Notifications And Performing The Ordered Service, Including On Non-Working Days, Sundays And Holidays; B) Was Punctual - Response Time To Notification (Transport Order): In Urgent Cases - Immediately, In In Other Cases - Within The Period Agreed With The Ordering Party, C) Had A Means Of Sanitary Transport: - Meeting All Sanitary And Technical Requirements Specified, Inter Alia, In Chapter 8 Of Order No. 77/2015/Dsoz Of The President Of The National Health Fund Of November 19, 2015 On Defining The Conditions For The Conclusion And Implementation Of Contracts For The Provision Of Health Services Such As: Primary Health Care; - Performed The Service With A Means Of Transport, The Technical Condition Of Which Will Ensure Full Safety Of The Transported Persons And The Appropriate Temperature Adapted To The Season (Car Equipped With Air Conditioning); And Administration For The Use Of Traffic Signals; - Operated By A Team Composed In Accordance With Applicable Regulations And Ordinances Of The President Of The National Health Fund; 10) The Means Of Transport Are Technically Efficient, Have Current Technical Inspections And Have Valid Ac, Oc And Nw Insurance; 11) The Contractor Declares That He Meets All The Requirements And Undertakes To Undergo Inspections Regarding The Implementation Of The Ordered Service (Contract). 12) The Trip Order Should Be Documented On The Basis Of The Completed Time Card. 13) The Transport Of Patients Will Take Place At The Contractors Risk. The Contractor Shall Bear Full Responsibility For Damages Caused To The Ordering Party And Third Parties As A Result Of Non-Performance Or Improper Performance Of The Subject Of The Contract. 14) The Contractor Shall Be Obliged To Comply With The Personal Data Protection Act And All Other Regulations Concerning The Protection Of Medical Data; 16) Vehicles Used For The Performance Of Services Should Not Be Older Than 7 Years. 17) The Minimum Payment Period Is 21 Days, With The Proviso That The Payment Date Is One Of The Bid Evaluation Criteria.3. Estimated Number Of Km Achieved By The Employer In A Month: Depending On The Circumstances, The Average Of 500 Km/Month In The Field Of Sanitary Transport, I.E. 6,000 Km/12 Months.
