For Professional Services Physical Therapy Services Collaborates With School Personnel, Including Teachers, Psychologists, Other Related Service Personnel, And Family To Meet The Needs Of All Students Consults, Collaborates And Provides Interventions As Appropriate Through The In The Multi-Tiered System Of Supports Conducts Physical Therapy Screenings Using Appropriate Screening Instruments Administers Formal And Informal Physical Therapy Assessments Based On Individual Student Needs, Including Appropriate Standardized Testing For Evaluations Observes Students In The Educational Environment Analyzes And Interprets Information To Make Recommendations Regarding The Need For Physical Therapy Services In The Educational Setting Prepares Evaluation Summary And Reports In A Timely Fashion Develops Present Level Of Educational Performance (Pleps), Goals And Benchmarks For Individualized Educational Programs (Iep) Within The Powerschool Iep Program Maintains Therapy Notes And Logs Using Our Data Service Center (Dsc) System Reports Progress Toward Goals In Powerschool Iep Serves As Member Of The Iep Team Communicates Effectively To School Personnel And Parents Provides Physical Therapy School-Based Activities Commensurate With Students Abilities, Interests And Aptitudes Selects And Implements Evidence-Based Practices Which Support The Goals And Benchmarks Uses A Variety Of Equipment, Materials, Devices And Aids Manages The Facilities, Materials And Equipment, Including Assistive Technology, Necessary To The Delivery Of Services Adjusts Intervention Strategies Based Upon Student Performance
