Medical Practice Services. 1. The Subject Of The Contract Is The Provision Of Preventive Health Care To Employees Of Organizational Units Of Poczta Polska S.A. 2. The Services Will Be Provided In The City Of Toruń And The Counties: Toruń, Grudziądz, Chełmno, Rypin, Brodnik, Wąbrzeski, Golubsko-Dobrzyński, As Well As Candidates For Work In This Area, Staffed By The Regional Department Of The Hr And Training Center In Bydgoszcz And Other Hr Units At Poczta Polska S.A. 3. Expected Number Of Services/Examinations: In The Planned Period Of 3 Years, The Total Of Preliminary, Periodic And Control Examinations Will Be Performed By Approximately 1,190 Employees On Average. 4. During The Execution Of The Order, The Ordering Party, Depending On The Actual Needs, Will Have The Right To Adjust The Number Of Services/Tests As Part Of The Services/Tests Listed In Appendix No. 2 To The Tor, While Maintaining The Maximum Remuneration. 5. The Tests Will Be Carried Out At The Contractors Facility, Which Must Have At Least One Facility Within The Administrative Boundaries Of The Town Of Toruń. The Employer Allows For The Possibility Of Performing Some Diagnostic Tests And Specialist Consultations At The Subcontractors Facility, With The Proviso That All Tests And Specialist Consultations Necessary To Issue A Certificate/Certificate To The Employee Will Be Performed In The Same Town Where The Occupational Medicine Facility Of The Contractor Conducting The Tests Is Located.
