The Subject Of The Contract Is The Performance Of A General Construction Renovation Of The Diagnostic Laboratory Building Constituting An Auxiliary Health Care Building At The Municipal Hospital In Rabka-Zdrój, Together With Its Thermal Modernization. 2. This Contract Covers Only The Execution Of Construction Works In The Laboratory Building Marked In The Technical Documentation As Building No. 2. The Employer Informs That Building No. 1 (Isolation Rooms) Has Been Demolished. 3. The Works Constituting The Subject Of The Contract Include In Particular: • Works Related To The Replacement (Disassembly) Of, Among Others: Door Joinery, Window Joinery, Ceilings, Electrical Installation Devices, Mechanical Ventilation Devices, Air Conditioners, Water And Sewage Devices, Central Heating, Including White Assembly Devices, Partition Walls, Plasters, Ceramic Surfaces On Walls And Floors In Rooms, Floor Layers On The Ground, • Assembly Of The Water And Sewage System, • Assembly Of The Central Heating System, • Assembly Of The Supply And Exhaust Ventilation System With The Central Unit, • Assembly Of The Electrical System, Including The Low-Voltage One, • Execution Of Partition Walls, • Execution Of Plasters, • Execution Of Putty On Partition Walls,• Execution Of Floors With Insulation Of The Floor On The Ground, • Execution Of Non-Slip Ceramic Tiles, • Replacement Of Window And Door Woodwork, • Execution Of Roof Insulation, • Installation Of Flashings, Gutters And Downpipes, • Execution Of Building Insulation, • Execution Of The Facade With Glue On The Mesh With Finishing Silicone Puff, • Foundation Insulation With Layers Of Anti-Moisture Insulation, • Lightning Protection Of The Building, • Finishing Works (Priming, Painting, White Assembly), • Hardening Of The Area, Leveling With Cobblestones In Front Of The Entrance To The Building, • Furnishing Of Rooms.4. As Part Of The Order, The Contractor Will Also Perform Construction Works Consisting In Making All Passages, Openings And Installations In The Building Elements, Including The Connection Of The Photovoltaic Installation, Which Will Be Carried Out In A Separate Procedure. 5. A Detailed Description Of The Subject Of The Order Is Contained In The Design Documentation Constituting Annex No. 1 To The Swz. 6. The Material Scope: 1) The Material Scope Is Defined In The Design Documentation. 2) The Scope Of Works Includes The Complete Implementation Of The Project, In Accordance With The Specification Of The Order Conditions And The Design Documentation. The Contractors Tasks Include The Preparation Of As-Built Documentation, Including, Inter Alia, The Operating And Health And Safety Instructions And The Performance Of All Other Works Necessary For The Use Of The Facility, In Accordance With Applicable Law And The Purpose Specified By The Employer. With The Documentation And With The Currently Applicable Standards, Construction Law Together With Executive Acts And Other Applicable Regulations, In Particular The Construction Law Together With Executive Acts. The Deadline For The Completion Of The Subject Of The Agreement Is The Date Indicated In The Final Acceptance Report Of The Subject Of The Agreement, Signed By The Ordering Party And The Contractor.6) The Ordering Party Allows Shortening The Deadline For The Execution Of The Subject Of The Agreement. And The Offer Cost Estimate No Later Than On The Date Of Signing The Contract. The Contractors Obligations Will Also Include: A) Proper Performance Of All Works Related To The Implementation Of The Subject Of The Contract To The Extent That It Enables The Use Of The Subject Of The Contract In Accordance With Its Intended Purpose, B) Organization And Protection Of The Construction Site Against Access By Third Parties, C) Carrying Out Construction Works In Accordance With The Principles Of The Code At Work And In Compliance With The Rules Of Occupational Health And Safety, Health And Environmental Protection And Fire Protection, D) Coverage Of The Costs Of Electricity And Water Consumption, E) Coverage Of The Costs Of Switching Off Or Damage To Media During Construction Works, F) Coverage Of The Costs Of Surveying Services, G) Covering The Costs Of Preparing The As-Built Documentation, Including The Geodetic As-Built Inventory, H) Covering The Costs Of Tests, Examinations And Expert Opinions (If Necessary) And Expenses Related To The Organization Of The Final Technical Acceptance Of Construction Works; 8. The Conditions For The Performance Of The Contract And The Scope Of The Contractors Obligations Have Been Included In The Draft Provisions Of The Public Procurement Contract Constituting Appendix No. 8 To Swz.9. Additional Information On The Terms Of The Contract: A) The Works Will Be Carried Out On The Premises Of The Hospital, Therefore - The Contractor Will Ensure Collision-Free Continuity Of The Hospitals Operation During The Performance Of The Works, Secure All Entrances And Access Routes In Accordance With Applicable Regulations (Roofing, Covers, Nets) Information On Security Measures Will Include It In The Bioz Plan, Which Will Be Prepared And Presented To The Employer Prior To The Commencement Of The Works. Any Works Interfering With The Normal Operation Of The Hospital Should Be Agreed With The Ordering Party (In Particular, The Performance Of Noisy Works Related To Heavy Dustiness Should Be Carried Out In A Way That Is Not Inconvenient For Patients And At Times Agreed With The Ordering Party).
