1. The Subject Of The Contract Is: The Provision Of Car Sanitary Transport Services By The Contractor For The Healthcare Complex In Oświęcim In The Field Of Round-The-Clock Sanitary Transport.2. The Transport Of People For The Zoz In Oświęcim Must Meet The Requirements Of The National Health Fund And Include, In Particular, The Transport Of The Ordering Partys Patients: - Between Departments Due To Their Location In Different Buildings, - To Other Medical Entities, - Transport Of Patients For Consultations, Diagnostic Tests And/Or Driving Patients Home And Transporting (Carrying In) The Patient By The Contractor, - Transport Of Blood And Material For Diagnostic Tests To Other Facilities. The Number Of Courses Depends On The Current Needs Of The Ordering Party Indicated In The Ordering Partys Order. A Detailed Description Of The Subject Of The Contract And The Requirements For Its Implementation Are Included In Appendix 1A To The Swz. The Employer States That The Total Estimated Number Of Kilometers To Be Performed For The Employer Will Be Approximately 170,800 Km And The Total Estimated Number Of Hours Of Work To Be Performed For The Employer It Will Be About 5500 H.3. The Contractor Undertakes To Indicate A Replacement Employee Who Will Not Be Allowed By The Employer To Perform His Duties Due To Non-Compliance With Applicable Law In The Scope Of Services Provided Or In The Event Of Reporting To Work In A Condition That May Disrupt The Order Of Work (I.E. In A Condition Indicating For The Consumption Of Alcohol Or Other Drugs). The Ordering Party Informs The Direct Supervisor Of The Employee About The Situation.5. The Contractor Should Have An Insurance Liability Insurance, Confirming That The Contractor Is Insured Against Civil Liability In The Scope Of Activities Related To The Subject Of The Contract And Provide A Copy Of The Policy Or Other Document Confirming Insurance Until The Date Of Signing The Contract For The Guaranteed Amount Of Not Less Than Pln 500,000.00.
