Complex Service Needs - Service Delivery Model Review And Continuous System Improvements - Disability Services Is Seeking A Contractor To Perform The Following Services/Deliverables: - Execute A Complex Service Needs Framework For 30 Service Delivery Model Reviews Involving Individuals With Highly Complex Needs; And - Create An Action Plan Based On The 11 System Improvements Identified From An Analysis Of 12 Service Delivery Model Reviews For Individuals With Complex Needs Previously Completed; And - Create Profiles For A Range Of Complexity Of Disability Services Clients And Provide A Detailed Description And Analysis Of That Profile; And - Provide An Executive Final Report That Articulates The Current State Of Service Delivery Models For Individuals With Complex Needs Within The Pdd Program, And Identifies Recommendations For Continuous Improvement For The Delivery Of Quality Services To This Population.
