POULTRY FOR DELIVERY POULTRY TO BE DELIVERED TO TORRANCE STATE HOSPITAL FOR PERIOD JULY TO SEPT 2022. The Department of Human Services, Torrance State Hospital is soliciting a supplier to provide Poultry items as described in the attached specifications. Bids must be submitted electronically. Bid will be awarded on lowest price per line item. This is for the delivery of poultry products for the period of JULY - SEPT 2022. Bidders will only bid on the acceptable brands, pack size and codes as specified. Bid awarded on lowest price per line item basis. Delivery dates are specified within the item text in accordance with facility storage space allotment and must be adhered to by the awarded vendor. Quantities listed are estimates and may be increased or decreased based on the needs of the facility. Alternate deliveries may be arranged, as required. Quantities may be adjusted at the request of the dietary department to meet the needs of our consumers. Complete and electronically return attached to your bid the Reciprocal Limitations Act Requirements Form. The Attached Reciprocal Limitations Act Requirements Form needs filled out in its entirety and submitted with your bid. ALSO THE NEW FORM: WORKER PROTECTION AND INVESTMENT CERTIFICATION FORM MUST BE SUBMITTED. Attachments to the Reciprocal Form will not be accepted. (applicable portions of pages 3 & 4). Failure to submit bid as described above may result in your bid being rejected as non-responsive. Delivery to be made the day specified, between the hours of 8:00 am and 11:45 am. Delivery not made on a state holiday. The complete specifications for the items are attached; the brand & manufacturer that will be accepted is listed. If an approved brand/manufacturer is not listed on the attached specs, the hospital will consider other brands-please indicate the manufacturer you are bidding on. These alternate brands will only be considered if no brand is listed on the state specs. Always bid on the unit of measure listed.
