SUPPLY OF MEDICAL GASES (TWO) 2 CONTRACTUAL YEARS • Copy of a valid Tax Compliance Certifi cate • Proof of current registration with Public Procurement Commission in the categories of Medical Equipment and Supplies • Completed and signed Bid Submission Form • Bid Security – J$121,689.56. The Supplier must be capable to provide the quantity of medical gases needed by the hospital to maintain as buff er stocks or assurance of availability of medical gases at any time. Provide immediate replacement of full cylinder with defect, to conduct monthly inspection and inventory of tanks with the assigned staff of the hospital for monitoring purposes.
