Monitoring Consultancy Program Management 1 Background Despite Substantial Progress The Deaths Of Children Under 5 Remain A Major Public Health Problem In The West And Central Africa Region Overall 2016 Statistics Show That 15 000 Children Under The Age Of 5 Die Every Day Of Which 46 7 000 Newborns Die Within 28 Days Of Birth The West And Central Africa Region Continues To Be The Region With The Highest Rate Of Under Five Mortality 95 Per 1 000 Live Births And The Highest Maternal Mortality Rate Of The World Recent Results From Mics Country Surveys Show That They Need To Double Their Efforts Progress In Accelerating Efforts Will Have To Be Made Otherwise The Country Risks Not Meeting The Sdgs And Ending Deaths By 2030 In Gabon Vaccine Preventable Diseases Undergoing Eradication Control Or Elimination Receive Active Surveillance And Are Reported On A Weekly Basis Poliomyelitis Measles Yellow Fever And Neonatal Tetanus In The Countries Of The Region The Low Capacity To Provide Quality And Equitable Care And The Scaling Up Of High Impact Interventions In Neonatal Infant Maternal And Adolescent Health Contributes To The Current Situation Especially In The Poorest Communities But The Main Reason For The Poor Performance Of The Health System Is The Mismatch Between Knowledge Policies And Actions That Manifests Itself In A Multitude Of Bottlenecks In Supply And Demand Including Quality Justification Unicefs Priorities For The West And Central Africa Region Stem From Its 2018 2021 Strategic Plan Aimed At Achieving Tangible Results For The Benefit Of Children In The Area Of Health Priorities Are Linked To The Following Key Results I Reduce The Inequity Gap In Immunization Through Polio Eradication And Reinforcement Of Routine Immunization Ii Save More Newborns And Iii Reduce The Gap In The Management Of Killer Diseases In Children Under 5 Years Of Age A Recent Who Analysis Has Shown That Countries In The West And Central Africa Region Such As Gabon Have Not Been Able To Meet The 15 Target Set By The Abuja Declaration About 16 Years Ago It Is Known That The Availability Of Funding Is A Crucial Factor That Determines The Performance Of A Health System In Sub Saharan Africa Public Health Spending Is Still Too Low To Ensure Equitable Access To Essential Health Services And Interventions In Fact Health Sector Budgets Oscillate Around 7 On Average In These Countries With Stagnation Of 4 To 5 Over Several Years In The Field Of Immunization All Countries In The Region Have Joined The Strengthening Of The Vaccine Initiative Launched Since 1974 And Have Established Expanded Programs Of Immunization Epi Which Are More Or Less Autonomous Entities Of The Ministries Of Health Health Whose Mission Is To Contribute To Better Child Survival By Reducing Morbidity And Mortality Attributable To Vaccine Preventable Diseases Upper Middle Income Countries With The Exception Of Cape Verde Regularly Face Challenges In Funding These Immunization Programs Despite The Commitment Of Governments To Programs With Budget For Immunization Among These Countries The Situation In Gabon Seems Worrying Despite The Fact That The Regional Certification Commission For Africa Has Obtained The Status Of A Polio Free Country According To Joint Who Unicef Estimates Of 2017 Vaccination Coverage In Gabon For The Third Dose Of Pentavalent Vaccine Has Stagnated To 75 Far From The 90 Required By The World Health Organization Who This National Average Hides Disparities At The Regional Level It Is Noted That Six Out Of Ten Health Regions Have Achieved A Vaccination Coverage Penta3 Greater Than Or Equal To 80 And That Two Health Regions Have The Lowest Vaccine Coverage In Penta 3 Below 57 Similarly Some Health Departments Remain Silent About The Surveillance Of Vaccine Preventable Diseases In View Of The Available Statistics And Despite The Governments Efforts To Provide The Epi With Budgetary Resources It Is Clear That Nearly 25 Of Surviving Children About 16 957 Ames Still Escape Vaccination In The Continuation Of These Efforts And Taking Into Account The Project Of The Operationalization Of Health Departments Promoted By The Government There Is An Urgent Need To Analyze The Budget Of The Ministry Of Health In Order To Identify Reallocation Possibilities Certain Budget Lines For Immunization At Both Central And Operational Levels The Health Department Is The Operational Unit For Implementing Primary Health Activities Including Immunization The Availability Of Resources For The Implementation Of Activities At This Level Would Improve The Performance Of The Immunization Program Ministry Of Health Budget Analysis An Important Step In The Epi Planning Process It Provides Essential Fi Nancial Information To Achieve At Least 90 Immunization Coverage At National Level Immunization Services Can Only Realize Their Potential To Improve The Health Of Children If They Have Reliable And Sufficient Fi Nancing It Is Widely Recognized That Strategic Planning For Immunization Involves Credible Information On Levels And Destinations Of Expenditure Sources Of Fi Nancing Fi Nancing Deficits And Future Needs 2 Overall Objective The Main Objective Of This Activity Is To Contribute To An Improvement Of Public Funding For Vaccination Through The Analysis Of The Budget Of The Ministry Of Health And The Development Of Scenarios For The Implementation And The Calculation Of The Costs Of The Expanded Program On Immunization 3 Specific Objectives Identify Bottlenecks In Immunization Financing Through An Analysis O Analysis Of The Level Of Immunization Funding Vaccines And Operational Activities In The Budget Of The Ministry Of Health In Comparison With Other Budget Lines O Analysis Immunization Program Budget Trends Compared To
