1) The Subject Of The Order Is The Provision Of Psychiatric And Psychological Services As Part Of The Project Titled: Bridges Over The Shadow - A Psychiatric Rehabilitation Program For Residents Of The Zduńska Wola District: A) Qualifying Psychiatric Counseling - 396 Hours; B) Psychological-Qualification Counseling - 384 Hours; C) Conducting Individual Skills Training In Dialectical-Behavioral Therapy - 2,280 Hours; D) Conducting Group Skills Training In Dialectical-Behavioral Therapy - 1,824 Hours; E) Conducting Individual Therapies For 95 People - 1,140 Hours; F) Conducting Group Therapies For 95 People. – 480 Hours; G) Conducting Supervision For Project Employees – 54 Hours; H) Conducting Stress Reduction Training For 190 People. – 192 Hours;I) Conducting Relaxation Training For 190 People. – 192 Hours;J) Conducting A Final Visit For 380 People. – 380 Hours2) The Estimated Number Of Tips May Change. The Ordering Party Reserves That It Will Not Be Obliged To Award The Contractor An Order In The Full Amount Of The Above-Mentioned Amount. The Indicated Number Of Advice In The Event Of Circumstances For Which The Ordering Party Is Not Responsible (E.G. Random Events Among The Project Participants, Lower Scale Of Interest In Project Support Among Participants Than Originally Assumed By The Ordering Party) - Option Right.3) The Ordering Party Reserves The Right To Commission The Contractor To Provide Advice To A Larger Number Of Participants (Up To 10%) In The Event Of The Need To Increase The Material Scope Of Services Constituting The Subject Of The Order Resulting From The Increased Interest Of The Participants In The Project, Provided That The Contractor Will Duly And Diligently Perform The Existing Basic Order, In Particular In The Absence Of Complaints/Comments From Participants - Change Allowed In Art. 455 Section 2 Pzp.
