The Subject Of The Order Is The Provision Of Veterinary Services Consisting In The Treatment Of Wild And Homeless Animals From The Area Of ​​The City Of Leszno That Have Suffered Injuries As A Result Of Accidents Or Other Random Events, Castration Or Sterilization Of Owner Cats Belonging To The Residents Of The City Of Leszno And Ensuring 24-Hour Readiness To Provide Emergency Services.2.Designation Of The Subject Of The Contract According To The Common Procurement Dictionary Cpv: 85200000-1 - Veterinary Services3.The Scope Of Services Includes Admission, Treatment, Diagnosis, Hospitalization, Euthanasia In Medically Justified Cases And Cooperation With City Services, Police, City Guards And Animal Shelters. A Detailed Procedure Is Set Out In Annex No. 1 To The Draft Contract According To Annex No. 8 To The Swz.4.As Part Of The Monthly Lump Sum Remuneration For Readiness To Provide Veterinary Services 7/24/365 (7 Days A Week, 24 Hours A Day, 365 Days A Year), The Contractor Will Ensure: 1) 24-Hour Telephone Availability (Or Other Accountable Form Of Contact) And Readiness To Intervene 7 Days A Week, Also On Public Holidays, 2) Immediate Intervention In Cases Emergencies, Such As Road Collisions Involving Animals, Aggression, Sanitary Or Epidemiological Threats, 3) Access To The Place Of The Incident In The City Of Leszno In Cases Where The Transport Of The Animal By Municipal Services Is Not Possible, 4) 24-Hour Readiness To Admit The Animal To A Treatment Facility, 5) Maintaining And Providing The Ordering Party With Documentation Of Each Intervention, Including A Description Of The Case, Diagnosis, Measures Used And Course Of Treatment On The Terms Indicated In Annex No. 1 To The Draft Contract According To Annex No. 8 To Swz.5.The Contractor Will Provide The Services With The Participation Of Personnel With Appropriate Qualifications And Professional Authorizations And Will Provide Appropriate Technical Resources, Diagnostic Equipment And Medicines.
