1. The Subject Of The Order Is The Provision Of Daily Stay Services For Min. 10 Seniors (People Over 65 Years Of Age), Including People With Disabilities, Lonely People And People Who Need Support In Everyday Functioning. The Services Are Addressed To Residents Of The Wyśmierzyce Commune. 2. Services Will Be Provided In The Białobrzegi Poviat In A Facility Functioning As A Day Support Center (E.G. Day Care Home, Day Social Welfare Home) Offering Services Within The Meaning Of Art. 51 Section 1 And 4 Of The Act Of 12 March 2004 On Social Assistance Adapted To The Needs Of Older People And People With Disabilities. The Services Will Be Provided 5 Days A Week, Excluding Public Holidays (E.G. Monday To Friday), 9 Hours A Day (E.G. 7:30 A.M. - 4:30 P.M.).3.The Contractor Bears Full Responsibility For Organizing And Conducting The Participant Recruitment Process.4.The Recruitment Must Be Carried Out Based On Transparent Regulations, Procedures And Objective Criteria Developed Independently By The Contractor, After Their Prior Approval By The Ordering Party.5.The Recruitment Procedure Must Take Into Account Priority Admission For The Most Needy People (Including People With Diagnosed Disabilities, People Running A Single Household And People Benefiting From The Support Of The Social Assistance System).6.As Part Of The Remuneration For The Implementation Of The Subject Of The Order, The Contractor Is Obliged To Provide Each Participant With The Following Minimum Package Of Services:● Day Care: Ensuring A Safe And Comfortable Stay In The Facility Under The Constant Supervision Of Qualified Staff (Manager, Caregiver For The Elderly / Occupational Therapist);● Transport: Organizing And Implementing Transport Of Participants With Limited Mobility (Door To Door) From Their Place Of Residence To The Facility And Taking Them Back After The End Of Classes, Using Vehicles Adapted To Transport People From Disabilities.● Activation And Integration Classes (Basic Schedule) The Contractor Is Obliged To Prepare And Implement A Permanent Monthly Schedule Of Classes, Including:○ Fitness And Movement Classes: A Minimum Of 2 Hours A Week Per Person (Conducted By A Physiotherapist, Rehabilitator Or Another Person With Qualifications In The Field Of Physical Culture/Motor Rehabilitation).○ Occupational Therapy And Social Skills Training: A Minimum Of 5 Hours A Week Per Person (Conducted By A Therapist Occupational Therapist Or Psychologist).○ Psychological Support: A Minimum Of 2 Hours A Week Per Person (Conducted By A Psychologist).○ Educational And Cultural-Educational Activities: A Minimum Of 8 Hours A Week (E.G. Lectures, Memory Training, Computer Classes).○ Hairdressing And Beauty Services: Providing Each Participant With Access To Basic Hairdressing Services Or Beauty (Cosmetic) Treatments Carried Out On Or Outside The Facility, With A Frequency Of At Least Once Every 6 Weeks.
