1. The Subject Of The Order Includes Successive Deliveries Of: Operational Gowns; Surgical Gowns, Surgical Drapes, Disposable Bed Linen And Protective Pads, As Well As Protective Equipment. The Assortment And Quantities, As Well As The Quality Requirements In Relation To The Subject Of The Order Are Detailed In Annex No. 1 To The Swz (Assortment And Quantity Specification).2.The Subject Of The Order Is One Task Divided Into 3 Packages. The Ordering Party Allows Submitting An Offer For The Selected Package(S), While The Full Range Of Products Is Required Within The Package, But Does Not Allow Submitting Offers For Selected Items In The Package.3.By Delivery, The Ordering Party Understands Periodic Deliveries During The Contract Period In Accordance With The Ordering Partys Orders Tailored To Its Current Needs.4.The Ordering Party Reserves The Right To Purchase A Smaller Quantity Of Products.5.During The Execution Of The Contract, The Ordering Party May Make Quantitative Transfers Between Individual Assortment Items Within The Limits Of The Value Of The Concluded Contract For Each Package.6.The Offered Subject Matter Of The Contract Must Meet The Legally Required Conditions Allowing Its Use. It Must Have Documents Authorizing Its Use In Health Care Facilities In The Republic Of Poland, In Accordance With The Requirements Specified In The Act Of April 7, 2022 On Medical Devices And The Implementing Regulations To This Act, I.E. A Declaration Of Compliance With The Ce Essential Requirements, Instructions For Use Of The Product, A Document Confirming That The Offered Medical Device Has Been Notified/Entered Into The Register Of Medical Devices And Entities Responsible For Their Introduction To The Market Or That A Notification Has Been Made About Product Pursuant To Art. 58 Of The Medical Devices Act. At The Same Time, The Contractor Undertakes To Submit The Above-Mentioned Documents At The Ordering Partys Written Request At The Place And Date Specified By The Ordering Party.7.The Contractor Ensures The Validity Of Documents Authorizing The Delivered Products For Circulation And Use For The Duration Of The Contract.8.It Is Required That Each Individual Item Of The Offered Assortment Is Provided With A Commercial Label In Polish, And Collective Packaging Should Be Provided With The Same Label. In Addition, The Commercial Label Should Contain At Least The Following Information: Type And Name Of The Product, Manufacturers Name, Code Number, Size, Batch Number, Expiration Date9.The Offered Assortment Must Meet The Requirements Of The Ordering Party Specified In The Description Of The Subject Of The Order And Fully Meet The Functional Requirements Appropriate For These Products In Accordance With Their Intended Use.10.The Contractors Obligation Is To Secure The Liquidity Of Supply Of The Products Covered By The Contract For The Entire Duration Of The Contract.11.Orders Will Be Placed Electronically Or By Fax, Depending On The Current Needs Of The Ord
