The Subject Of The Order Is The Delivery Of An Electromyograph With An Electrostimulation Function (1 Set), Along With Its Commissioning And Conducting The Necessary Training.1. The Medical Device Offered By The Contractor Should Have The Status Of A Medical Device Placed On The Market And For Use In The Territory Of The Republic Of Poland, In Accordance With Polish And European Union Law, And In Confirmation The Contractor Will Submit A Declaration (Included In The Identification And Price Form) That It Has A Set Of Documents Allowing The Offered Subject Of The Order To Be Placed On The Territory Of The Republic Of Poland In Accordance With The Act Of April 7, 2022 On Medical Devices (Journal Of Laws Of 2024, Item 1620) And Regulation (Eu) 2017/745 Of The European Parliament And Of The Council Of 5 April 2017 On Medical Devices (Oj Eu L 117 Of 05/05/2017, P. 1, As Amended) - I.E. The Ce Marking And The Eu Declaration Of Conformity, And If Applicable - The Entitys Certificate Notified, And That Copies Of These Documents Certified As True Copies Of The Originals Will Be Presented At Each Request Of The Ordering Party, And In Each Case They Will Be Delivered Together With The Delivery Of The Subject Of The Order, Which Will Be Confirmed In The Acceptance Report.2. The Offered Medical Device Must Meet The Minimum Parameters And Technical And Functional Requirements Specified By The Ordering Party In The Identification And Price Form Constituting Annex No. 2.1 To Swz.3. The Offered Medical Device Must Be Brand New With A Production Date No Older Than 2026. Warranty: Minimum 24 Months - Counted From The Date Of Signing The Acceptance Protocol Without Comments.
