The Terms Of Execution Of The Order Are Included In The Draft Contract Constituting Annex 4 To The Swz And In This Specification Of Terms Of Order. The Contractor Is Obliged To Complete The Order On The Terms And Conditions Described In The Draft Contract And Swz With The Care Required In This Type Of Activity And In Accordance With The Principles Of Professionalism And Applicable Regulations. 3. The Ordering Party Reserves The Right To Exercise The Option Right. The Exercise Of The Option Right Will Consist In Increasing By 80% The Number Of Services Included In The Description Of The Subject Of The Order In The Event Of Exhaustion Of The Amount Allocated For The Implementation Of The Basic Order Referred To In § 5 Section 1 Of The Draft Contract, Subject To § 5 Section 6 Of The Draft Contract. 4. The Option Right Referred To In Point 3 (Above) Will Be Performed On The Same Terms And Conditions As The Basic Order. 5. The Unit Prices Of Additional Tests And Diagnostic Consultations Under The Option Law Will Be Identical To Those Of The Basic Order. However, The Ordering Party Reserves The Right To Check The Proposed Prices On The Generally Available Medical Services Market And, In Justified Cases, Request A Reduction Of The Proposed Prices. 6. The Ordering Party Reserves That The Part Of The Order Defined As The Option Right Is An Entitlement And Not An Obligation Of The Ordering Party. The Option May, But Does Not Have To, Be Exercised, Depending On The Ordering Partys Needs And As A Result Of Its Instructions In This Regard. Failure To Perform The Order In This Respect Will Not Give Rise To Any Claims On The Part Of The Contractor Against The Ordering Party. 7. Exercising The Option Right Will Not Require The Execution Of An Annex To Nn. Contract, Will Be Based On Written Information Sent To The Contractor About The Activation Of The Option Right, And Then A Written Order(S). 8. The Contractor Will Declare To The Ordering Party That He Has A Document Confirming That The Contractor Is Insured Against Civil Liability In The Scope Of His Business In The Amount Of Min. The Amount Of Eur 100,000 In Relation To One Event And Eur 500,000 In Relation To All Events - In Accordance With The Regulation Of The Minister Of Finance Of April 29, 2019 (Journal Of Laws 2025.272); 9. If The Contract Is Signed, The Contractor Is Obliged To Ensure The Validity Of The Insurance Policy In The Scope Of Its Activities Related To The Subject Of The Contract And Undertakes To Ensure Its Validity Throughout The Duration Of The Contract. 10. The Subject Of The Contract Is The Provision Of Medical Services In The Field Of Occupational Medicine And Other Medical Services By The Contractor To The 11Th Military Economic Department And Institutions Included In The List Constituting Annex No. 2 To The Draft Contract And Annex No. 5 To The Swz: 1) Performing Examinations And Issuing Decisions In The Field Of Occupational Medicine For Soldiers And Military Employees; 2) Determination Of Blood Group With Entry In Documents; 3) Performing Tests For Drivers Of Motor Vehicles, Including Drivers Of Emergency Vehicles; 4) Performing Laboratory And Radiological Tests
