1. The Subject Of The Order Is The Supply Of Endoprostheses, Implants, Titanium Plates, Screws And Microscrews For The Needs Of The Otolaryngology Department With The Sub-Department Of Maxillofacial Surgery Of The Provincial Hospital In Elbląg, The Detailed Description, Range And Quantities Of Which Are Specified In Annex No. 1 To Swz.2.The Contractor Will Lend The Ordering Party Reusable Instruments Necessary For The Implantation Of Implants, Along With Containers For Their Storage And Sterilization, For The Duration Of The Contract. (The Requirements And Scope Of The Borrowed Equipment Are Described In Annex No. 1 To The Swz). The Equipment Will Be Loaned On The Terms Specified In The Loan Agreement, The Template Of Which Is Attached As Appendix No. 5 To Swz.3. The Contractor Will Provide Training For Medical Staff (5 Doctors And 2 Nurses Selected By The Ordering Party) Of The Otolaryngology Department With The Maxillofacial Surgery Sub-Department In The Form Of A Practical Course Or By Participating In Lectures On Topics Related To The Ordered Products.
