Tenders Are Invited For Providing Care Services At Home For Residents Of The Municipal Social Welfare Center In Wejherowo

The Subject Of The Order Is Provision Of Care Services At Home For Residents Of The Municipal Social Welfare Center In Wejherowo.2.The Subject Of The Order Includes Assistance In Meeting Everyday Life Needs, Hygiene Care, Care Recommended By A Doctor To The Extent That Does Not Require Specialized Knowledge And Competences, And, If Possible, Ensuring Contacts With The Environment. Services Are Provided In Accordance With Art. 17 Section 1 Point 11, Art. 50, Art. 50A And Art. 50B Of The Act Of March 12, 2004 On Social Assistance (Consolidated Text: Journal Of Laws Of 2026, Item 639, As Amended) In The Period From January 1, 2027 To December 31, 2027. The Services Are Variable And Depend On The Individual Needs Of People Entitled To The Service And The Current Care Situation.3.The Amount Of Benefits Expected During The Term Of The Contract: 1) The Estimated Amount Of Care Services To Be Provided During The Term Of The Contract Is 6,252 Hours Per Year; 2) The Ordering Party Specifies The Minimum Number Of Hours To Be Provided During The Term Of The Contract At 4,000 Hours Per Year. The Minimum Amount Is The Guaranteed Number Of Services For The Contractor;3) The Expected Number Of People Covered By The Services Is Approximately 16 People Per Month. The Values ​​Provided Are Estimates And May Change During The Term Of The Contract, In Accordance With The Actual Needs Of The Ordering Party.4.Variability Of The Number Of Hours And The Number Of People Under Care:1) The Actual Number Of People Covered By The Services And The Number Of Hours Of Care Services Will Depend On The Current Needs Of The Ordering Party And May Be Increased Or Decreased, Within The Limits Specified In This Tor And The Contract;2) Changes In The Dimension Of Services During The Execution Of The Order Cannot Lead To A Reduction Of The Contractors Remuneration Below The Value Corresponding To The Minimum Number Of Hours (4,000 H/Year).5.The Contractor Acknowledges That Assistance In The Form Of Services Is Provided On The Basis Of An Administrative Decision. When Granting Services, The Director Of The Municipal Social Welfare Center In Wejherowo Or Another Authorized Person Determines Their Scope, Period Of Implementation, Amount, Place Of Provision And Amount Of Payment For The Services.6.A Detailed Description Of The Subject Of The Order Is Set Out In Annex No. 6 To The Swz Detailed Description Of The Subject Of The Order.
