1. The Subject Of The Order Is The Successive Collection, Transport And Disposal Of Medical Waste Generated In The Facilities Of The District Health Center Sp. Z O.O. In Otwock, For A Period Of 24 Months From The Date Of Conclusion Of The Contract.2.The Order Includes Waste With Codes: 18 01 02*, 18 01 03*, 18 01 04, 18 01 06*, 18 01 08*, 18 01 09 And 18 01 10*, Produced In The Following Locations Of The Ordering Party: 1) Hospital - 05-400 Otwock, Ul. Batorego 44; 2) District And Specialist Clinic – 05-400 Otwock, Ul. Armii Krajowej 3; 3) District Clinic In Otwock–Świdra – 05-400 Otwock, Ul. Mickiewicza 8.3.A Detailed Description Of The Subject Of The Order, Including The Rules And Frequency Of Collection, Estimated Quantities, Requirements For Weighing And Documenting Receipt, And Requirements For Transport And Waste Disposal, Are Set Out In: 1) Annex No. 2 To The Swz - Description Of The Subject Of The Order; 2) Annex No. 3 To The Swz - Draft Provisions Of The Contract.4.The Estimated Maximum Total Weight Of Waste During The Order Execution Period Is 60.00 Mg (60,000 Kg) And Is The Upper Limit Of The Quantity Covered By The Agreement. The Ordering Party Guarantees The Execution Of The Order In An Amount Of At Least 54.00 Mg (54,000 Kg). Settlement Will Be Made According To The Actual Weight Of Waste Collected And Disposed Of And The Unit Price Per 1 Kg Indicated In The Offer, In Accordance With The Principles Specified In The Swz And The Agreement.
