1. Type Of Order – Service.2.The Subject Of The Order Is The Provision By The Contractor Of Comprehensive Cleaning Services For Rooms In Buildings Located At Ul. Głogowska 25C In Opole, Ul. Oleska 127 In Opole, Ul. Damrota 7 In Opole And At Ul. Reymonta 14 In Opole - In The Parts Occupied By The Provincial Labor Office In Opole.3.The Detailed Scope Of The Service Is Included In: 1) Annex No. 2 To The Ppu - Annex No. 9 To The Swz - Scope Of Activities And Cleaning Products Used, 2) Annex No. 3 To The Ppu - Rules For The Implementation Of The Cleaning Service4.The Subject Of The Order Includes A Total Area To Be Cleaned Of 2,680.22 M² In Accordance With The List Of Areas Covered By The Cleaning Service Constituting Annex No. 1 To The Ppu, According To The Following Division: 1) Area Of 2,680.22 M² Cleaned From Monday To Friday (All Locations), 2) Area Of 128.25 M² (Ground Floor Rooms At Ul. Damrota 7) Cleaned Every Second Saturday Starting From January 9, 2027, Excluding Holidays And Days On Which The Provincial Labor Office In Opole Is Closed. If The Extension Option Is Used, The First Saturday Covered By The Service In 2028 Is Saturday, January 8, 20285.The Ordering Party Envisages The Possibility Of Applying The Option Right: 1) Consisting In Extending The Period Of Service Provision For The Period From January 1 To December 31, 2028 On The Terms Described In § 2 Section 3-6 Ppu (Hereinafter Referred To As The Extension Option),2) Consisting In Reducing The Number Of M2 Covered By The Service Or Limiting The Days On Which The Service Will Be Provided On The Terms Described In § 2 Section 7-11 Ppu (Hereinafter Referred To As The Reduction Option),3) Consisting In Increasing The Number Of M2 Covered By The Cleaning Service On The Terms Described In § 2 Section 12-16 Ppu (Hereinafter Referred To As The Increase Option). Details Regarding The Exercise Of The Option Right Are Described In Ppu.6.The Contractor Undertakes To Perform All Services Using Its Own Means, Materials, Machines And Devices.7.The Ordering Party Requires The Contractor To Have At Least 5 People Capable Of Executing The Order (Providing The Service) And A Coordinator.8.The Ordering Party Requires That The Contractor Be Insured Against Civil Liability Arising From His Business Activity For An Insurance Sum Of At Least Pln 250,000.00 Throughout The Entire Duration Of The Contract.9.The Ordering Party Requires From Contractors Or Subcontractors That Persons Performing Activities Involving The Physical Performance Of Services Within The Scope Specified In Detail In Annex No. 2 And 3 To The Ppu Are Employed Under An Employment Contract (Does Not Apply To The Coordinator).10.The Method Of Verifying The Employment Of The Persons Referred To Above, The Ordering Partys Control Rights And The Sanctions For Failure To Meet These Requirements Are Specified In Ppu.11.Approximate Number Of The Ordering Partys Employees In Each Location: 1) Ul.
