The Subject Of The Order Is: 1) Delivery Of A Brand New Computed Tomography Device With Complete Equipment, Its Installation, Assembly And Commissioning In A Dedicated Laboratory Indicated By The Ordering Party, Together With The Implementation Of Adaptation Works, Adaptation Of The Laboratory For The Installation Of A New Device, In Accordance With The Manufacturers Installation Requirements And The Scope Approved By The Ordering Party, And Performance Of Measurements And Necessary Tests Related To The Commissioning Of The Computed Tomography Scanner In Accordance With Legal Requirements, 2) Disassembly And Disposal Of The Computed Tomography Scanner Used By The Ordering Party And Intended For Disposal With Serial Number 64476, 3) Conducting Training For The Ordering Partys Staff In The Field Of Technical And Clinical Maintenance Of The New Computed Tomography Scanner To The Extent Permitted By The Manufacturer To Be Performed By The User Independently, In Accordance With The Boundary Conditions - A List Of Required And Assessed Parameters Constituting Annex No. 9, A Contract Template Constituting Annex No. 4 And A Price Form Constituting Annex No. 3 To This Terms Of Reference.
