4.1.The Subject Of The Order Is Purchase Of An Ultrasound Scanner For The Municipal Health Center Of Sp Zoz In Puchaczów. 4.2.The Material Scope Of The Subject Of The Order Includes In Particular: 1) Purchase And Delivery Of 1 Piece Of Ultrasound Scanner, In Accordance With Annex No. 1 To The Swz, 2) Transfer Of The Subject Of The Order, Brand New (I.E. Unused, Unregenerated, Not For Exhibition, Not For Demonstration Purposes, Produced No Earlier Than In 2026) Of Good Quality, Complete, Free From Defects (Including Physical, Aesthetic, Constructional, Legal), In Accordance With Applicable Regulations In This Regard Provisions Of Law, Must Have A Ce Declaration Of Conformity/Ce Certificate, Entry In The Register Of Medical Devices And Entities Responsible For Their Introduction To The Market And Use In The Territory Of The Republic Of Poland, Guaranteeing The Safety Of Patients And Medical Staff.3) The Contractor Declares That The Offered Device Is A Medical Device Within The Meaning Of The Act Of April 7, 2022 On Medical Devices And Meets All The Requirements Specified In This Act And The Regulations Issued On Its Basis. 4) The Contractor Will Provide Free Instruction - Training To Medical Staff Indicated By The User In The Basic Operation And Operation Of The Subject Of The Contract. 5) The Contractor Will Install, Launch And Configure The Delivered Ultrasound Scanner At No Additional Cost. 6) Transport Of The Subject Of The Order To The Seat Of The Municipal Health Center Of The Independent Public Health Care Facility In Puchaczów 21-013 Puchaczów Ul. Partisan 9 Is The Responsibility Of The Contractor And Is At His Expense. During Transport, The Subject Of The Order Should Be Secured By The Contractor In A Way That Prevents Its Accidental Damage And Loss Of Its Properties And Parameters. 4.3.The Contractor Undertakes To Implement The Subject Of The Contract Taking Into Account - If Possible Taking Into Account The Specific Nature Of This Order - The Minimum Requirements To Ensure Accessibility For People With Special Needs, Referred To In Art. 6 Of The Act Referred To In Section 10.4.4.The Contractor Is Obliged To Provide, Together With The Ultrasound Scanner, The Documents Specified In Annex No. 2 To The Swz - Draft Contract.4.5.A Detailed Description Of The Subject Of The Order And The Technical Parameters Of The Subject Of The Order Are Specified In Annex No. 1 To The Swz, See Detailed Description Of The Subject Of The Order - Minimum Technical And Operational Requirements For An Ultrasound Scanner. All Requirements Specified In The Swz Are Minimum Requirements And Their Fulfillment Is Obligatory. Failure To Meet The Above-Mentioned Minimum Requirements Will Result In Rejection Of The Offer As Inconsistent With The Terms Of The Order Pursuant To Art. 226 Section 1 Point 5 Of The Public Procurement Law.
