The Subject Of The Order Is To Conduct Medical Consultations With A Doctor Specializing In Otorhinolaryngology (Hereinafter: Ent Doctor) For The Administrative Employees Of The University Of Life Sciences In Poznań (Project Participants). Medical Consultations With An Ent Doctor 3 32 96 Consultation Mode: Stationary At The Contractors Facility In The City Of Poznań. Additional Requirements: A. Establishing A Schedule With The Ordering Party With Information About Hours During Which The Services Provided By The Contractor Are Available To Himb.Receiving Applications And Arranging Meetings Within The Above-Mentioned Schedule, C.Confirming The Presence Of People Using Consultations For At Least 48 Hours Before The Scheduled Meeting D.Confirmation Of Consultations With Representatives Of The Project Department Of The University Of Life Sciences In Poznań In Order To Support And Select Appropriate Forms Of Adaptation For People Using The Consultations (If Necessary). Duration Of One Consultation: Minimum 30 Minutes For The First Consultation, Minimum 20 Minutes For The Second And Subsequent Consultation Total Number Of Consultations: 96 Consultations Planned. Minimum Number Of Consultations That The Ordering Party Expects To Carry Out: 32 At The Same Time, It Is Expected That Increasing The Total Number Of Consultations By 32. The Final Number Of Completed Consultations Depends On The Interest Of The Project Participants. Detailed Scope Of The Service: The Consultation(S) Is Intended To Diagnose The Professional Factor(S) Adversely Affecting The Health Of The Project Participant(S). The Consultation(S) Is Intended To Include In Particular: A.Checking The Health Condition Of The Person Using The Consultation Within A Given Medical Specialization, Including Carrying Out The Necessary Tests, B.Diagnosis Of Needs And Identification Of Factors That Threaten The Health Of People Present At Their Workplace, C.Issuing Recommendations On Adapting The Workplace To The Specific Needs Specified In The Needs Diagnosis, D.Recommendations On How To Deal With A Diagnosed Health Problem In The Context Of The Professional Position Held Order Date: Agreement For The Provision Of Services (From The Date Of Conclusion Of The Contract - April 2027) Settled Monthly Based On The Number Of Consultations Completed And The Invoice Issued.
