The Detailed Scope Of The Order And The Minimum Quality And Technical Requirements Are Specified In: Annex No. 1 - Description Of The Subject Of The Order And Annex No. 2 - Technical Parameters Form.3. Place Of Delivery And Installation: Family Doctors Clinic, Ul. Wyzwolenia 1, 47-450 Krzyżanowice.4. Main Cpv Code: 33112000-8 – Ultrasound, Echo And Doppler Equipment.5. The Order Is Implemented Under The Grant Project No. Fenx.06.01-Ip.03-0001/23 Support For Primary Health Care (Poz), Based On The Grant Agreement No. 012Ow/1106/Ii/2026 Of July 7, 20266. The Ordering Party, Using The Price Criterion With A Weight Of 80%, Specified In The Opz And The Technical Parameters Form Measurable Quality Requirements Relating To The Main Elements Of The Device, Heads, Software, Integration, Security, Service And Training, In Accordance With Art. 246 Section 2 Of The Pzp.7 Act. The Device Will Be Installed In The Family Doctors Clinic At Ul. Wyzwolenia 1 In Krzyżanowice And Will Be Used By The Ordering Party Throughout The Duration Of The Project Only To Provide Health Care Services Financed From Public Funds, Such As Primary Health Care. The Durability Period Is 5 Years From The Date Of The Last Payment To The Grantor; The Date Of The Last Payment Is The Date Of Debiting The Payment Account Of The Institution Transferring The Funds To The Grantor, In Accordance With The Grant Project Implementation Procedure.8. The Contractor Is Obliged To Cooperate With The Ordering Party In Documenting The Expense, Control And Audit, Implementation Of The Dnsh Principle And Mandatory Labeling Of Equipment, In Accordance With The Principles Specified In The Opz And The Draft Provisions Of The Contract.
