1. The Subject Of The Order Is The Supply Of A Type C Ambulance With A 4X4 Drive, Gvw Above 3.5 T, Together With Medical Equipment Intended For The Polish Baptism Monument Hospital In Gniezno, For Which The Ordering Party Is Applying For Funding From A Special-Purpose Subsidy From The State Budget To Finance Its Own Task In The Field Of Civil Protection And Civil Defense Included In Key Areas Of Activity Under The Civil Protection And Civil Defense Program For The Years 2025-2026. Detailed Technical Requirements Are Included In Annex No. 1A To The Swz - Description Of The Required Technical Parameters, Which The Contractor Is Obliged To Complete And Attach To The Offer.2.Option: 1) The Ordering Party Provides For The Possibility Of Exercising The Option Right Referred To In Art. 441 Of The Act Of September 11, 2019 - Public Procurement Law. 2) The Basic Order Includes The Delivery Of 1 Piece Of A Brand New Type C Ambulance With The Equipment Specified In The Description Of Required Technical Parameters - Annex No. 1A To The Swz. 3) Under The Option Right, The Ordering Party Reserves The Right To Order Additional Equipment Intended For The Delivered Ambulance, Which Is An Ultrasound Machine - 1 Piece, Meeting The Requirements Of The Description Of Required Technical Parameters - Annex No. 1A To The Swz, For Which The Contractor Will Be Entitled To A Separate Remuneration.4) The Contractor Is Obliged To Execute The Order Covered By The Option Right On The Terms Specified In The Agreement And At The Unit Prices Indicated In The Offer Form.5) The Exercise Of The Option Right Depends On The Partys The Ordering Partys Need To Equip The Ambulance With Additional Equipment And The Ordering Partys Possession Of Financial Resources Intended For The Implementation Of The Option.6) The Ordering Partys Exercise Of The Option Right Does Not Constitute An Amendment To The Contract Or The Conclusion Of A Separate Contract And Is Based On The Authorization Provided For In This Tor And The Contract.7) The Option Right May Be Activated By The Ordering Party By Submitting To The Contractor A Written Or Electronic Declaration On The Exercise Of The Option Right, Indicating The Scope Of The Ordered Equipment.8) The Declaration Referred To In Point 7) Is Submitted No Later Than On The Date Of Signing The Agreement. 9) Delivery Covered By A Properly Activated Option Will Take Place Within The Validity Period Of The Agreement In The Scope Of The Basic Order From The Date Of Receipt Of The Declaration.10) The Ordering Party Is Not Obliged To Exercise The Option Right, And The Contractor Is Not Entitled To Any Claims In This Respect.3.The Subject Of The Order Will Be Implemented As Part Of The Task Entitled: Delivery Of A Type C Ambulance With 4X4 Drive Is Financed Under The Civil Protection And Civil Defense Program In 2025-2026 For The Implementation Of Tasks In Area Ii Logistics Security And Ensuring Continuity Of Supplies Under Measure Ii.1.6.
